视觉电生理在糖尿病性视网膜病变中的应用研究

摘要 目的 探讨糖尿病患者的视觉电生理特征及其与眼底改变之间的相互关系。方法 采用国际标准法检测 30例 (6 0眼 )正常人和 33例 (6 6眼 )Ⅱ型糖尿病患者 (DiabetesMellitus,DM)的视网膜振荡电位 (Odillatrypoten tials,ops)和图形视觉诱发电位 (Pattenviualevoredpotential,PVEP)。 结果 DM患者OPS、PVEP与对照组之间有非常显著性差异 (P <0 .0 1) ,表现为OPS总幅值及P10 0 波幅值的降低 ,P10 0 波潜时值的延迟 ;DM组在眼底尚无病变前 ,其OPS、PVEP已出现异常 ,异常率随病变加重而增加 ,分别为 5 6 .0 6 % ,5 7.5 8% ,略高于眼底荧光造影术异常检出率 (5 4.0 9% )。结论 DM患者视功能损害早于眼底病变 ,这种损害是多层次、同时发生的 ,故应联合使用多种眼电生理检查法。 Objectives In order to investigate the relationship between the features of the visual electrophyciology and fundus change in diabetes mellitus.methods PVEP and OPS were examined in 30 normal individuals(60 eyes) and 33 patients (66 eyes) with Ⅱ DM.All individual′s visual electrophyciology were recorded accvrding to ISCVE standard.Results The amplitude of ops and p100 were significantly reduced and the latency of p100 was significantly delayed in the DM group.The ops and PVEP indexs were abnormal in non-DR.Those abnormalitied were increased with increasing extent to retinopathy from stage Ⅰ~Ⅲ In the DM group,the abnormal cletecbable rate of the amplitude of ops was 56.06% and the lateney of p100 was 57.58%,which was slightly higher than that of fundus fluorescin angiography(54.09%).Conclusions The functional lesion in DM appeared earlier than the visiable fundus chnages,the lesion were occured in differentl layers in same time.so ops and PVEP should be combined to use
