制动对兔膝外侧副韧带生物力学与形态学特性的影响 被引量:10

Effect of immobilization on biomechanics and morphology of lateral collateral ligament in the knee of rabbit

摘要 目的 研究制动对兔膝外侧副韧带生物力学及形态学特性的影响。方法 将 15只大白兔随机分为 3组 :实验组 10只 ,行右后肢膝关节全屈曲位内固定 12周 ;对侧对照组 10只 ,不作处理 ;正常对照组 5只。取外侧副韧带行生物力学测试和形态学观察。结果 制动 12周后 ,实验组外侧副韧带横截面积缩小 ,最大负荷、能量吸收明显减少 ,与对侧对照组和正常对照组比较差异有显著性 (P <0 .0 1或P <0 .0 5 )。最大应力、弹性模量、材料常数等材料特性指标也存在显著性差异。应力一应变曲线与对侧对照组和正常对照组比较呈分离状态。光镜下胶原纤维排列紊乱 ,韧带腓骨止点出现不规则溶骨性吸收 ,断裂方式全为腓骨止点处撕脱骨折。结论 制动不仅改变韧带骨组合体的结构特性 ,也影响韧带物质的材料特性。 Objective To elucidate effect of immobilization on biomechanics and morphology of lateral collateral ligament (LCL) in the knee of rabbits.Methods 15 New Zealand white rabbits were assigned to three groups Group Ⅰ, 10 rabbits were immobilized for twelve weeks; Group Ⅱ, same 10 rabbits served as contralateral control; Group Ⅲ, 5 rabbits were selected as age matched normal control. The right hindlimb of each experimental rabbit was surgically immobilized in full flexion (170°) by threaded pin internal fixation. The biomechanical testing,histological and ultrastructural evaluation were used.Results Cross section area,maximal loading,energy absorption,maximum stress,LCL elasticity modulus in Group I decreased as compared with those of other two groups. In addition, there was significant difference between the material constants X1 and X2 of Group I and those of other two groups (P< 0.01 ). Histologically, after immobilization for twelve weeks, arrangement of collagen fibers of LCL was disorganized under microscope. Fibulae insertion sites of LCL showed resorption of bone. The failure mode of LCL was by avalsion fracture of fibulae insertion sites.Conclusion Immobilization was not only altered the structural properties of ligament bone complexs in LCL, but also altered the material properties of ligament substances.
