中心静脉导管胸腔内留置后的护理风险干预

Nursing risk intervention on indwelling central venous catheter in pleural cavity

目的探讨护理风险干预对胸腔内留置中心静脉导管（CVC）引流积液术后安全性的影响。方法将2010年7月~2012年1月行中心静脉导管置管引流的66例结核性胸腔积液患者作为观察组,在引流管常规护理的基础上给予护理风险干预;回顾2009年1月~2010年6月仅给予常规护理的52例CVC置管引流的结核性胸腔积液患者作为对照组;比较两组患者引流管扭曲或折叠、堵塞、滑脱及穿刺部位继发感染等并发症的发生情况。结果观察组患者发生管道扭曲或折叠、堵塞及滑脱发生率分别为7.58%、4.55%和3.03%;对照组分别为21.15%、17.31%和15.3%,两组差异具有统计学意义（P〈0.05）;观察组穿刺部位感染的发生率为4.55%,也明显低于对照组的15.38%。结论实施护理风险干预措施,可减少中心静脉导管置管引流结核性胸腔积液术后并发症的发生,提高引流管安全性。

Objective To explore the effect of nursing risk intervention on the safety of drainage surgery of indwelling central venous catheter（CVC） in pleural cavity.Methods Sixty-six tuberculosis pleural effusion patients who received CVC drainage from 2010 July to 2012 January were selected as observation group.The observation group received nursing risk intervention in addition to the routine nursing.Another 52 tuberculosis pleural effusion patients who received CVC drainage from January 2009 to June 2010 were selected as control group.The control group only received routine nursing.The complications including drainage tube distortion or twisting,jam,slippage and puncture site infection were compared in the two groups.Results The rates of tube distortion or twisting,jam and slippage were 7.58%,4.55%,3.03%,respectively in the observation group and 21.15%,17.31%,15.3% in the control group.The differences of the two groups were statistically significant（P〈0.05）.The rate of puncture site infection in the observation group was 4.55%,significantly less than 15.38% in the control group.Conclusion The nursing risk interventions can reduce postoperative complications of CVC drainage surgery in tuberculosis pleural effusion,and improve drainage safety.
