摘要
目的探讨高血压与胰岛素抵抗和C-反应蛋白的相关关系,为高血压防治提供参考。方法对180例高血压患者和131例非高血压患者检测体质指数(BMI)、空腹血糖(FPG)、空腹胰岛素(Fins)、胰岛素抵抗(IR)、C-反应蛋白(CRP)、收缩压(SBP)和舒张压(DBP),作比较分析。结果高血压组的BMI、FPG、Fins、CRP和IR均明显高于对照组,差异有统计学意义(P〈0.05)。重度高血压患者的BMI、FPG、Fins、CRP和IR均明显高于中度高血压患者,而中度高血压患者的上述指标又明显高于轻度高血压患者,差异均有统计学意义(P〈0.05)。Pearson相关分析显示,SBP与CRP和IR分别呈正相关系(r=0.381,P〈0.05;r=0.523,P〈0.05),DBP与CRP和IR分别呈正相关系(r=0.328,P〈0.05;r=0.501,P〈0.05)。结论高血压与胰岛素抵抗和CRP密切相关,推测胰岛素抵抗和炎症反应可能参与高血压的进展。
Objective To study the relation of hypertension with insulin resistance and C-reactive protein, in order to provide clinical evidence for its prevention and treatment. Methods 180 cases with and 131 cases without hypertension were recruited in the study. Both in the two groups the body mass index ( BMI ) , fasting plasma glucose ( FPG ) , fasting insulin ( Fins ) , C-reactive protein ( CRP ) , systolic blood pressure ( SBP ) , and diastolic blood pressure ( DBP ) were measured. Results The values of BMI, FPG, Fins, CRP and IR in hypertension group were significantly higher than those in control group ( P〈0.05 ) . Those targets in severe and moderate hypertension were obviously higher than those in mild hypertension group ( P〈0.05 ) , as well as it was in the severe group than the moderate group ( P〈0.05 ) . The SBP level showed remarkably positive correlation with the CRP and IR (r=0.381, P〈0.05; r=0.523, P〈0.05 ) , and so did DBP level ( r=0.328, P〈0.05 ; r=0.501, P〈0.05 ) . Conclusions Hypertension is closely related to insulin resistance and CRP, and it is suggested that insulin resistance and inflammatory reaction may be involved in the hypertension progress.
