Application of CBP in the rescue severe phosgene poisoning with MODS

目的探讨CBP在急性重度光气中毒合并MODS中的临床应用。方法对1例急性重度光气中毒合并MODS患者应用CBP治疗前后临床症状、体征、动脉血气和影像学变化等临床资料进行回顾分析。结果该例患者在以机械通气、糖皮质激素为主的综合治疗无效后,采用CBP治疗,临床症状及体征迅速改善,动脉血气恢复正常,肺部的影像学变化明显好转。结论对急性重度光气中毒合并多脏器功能障碍的患者,常规治疗无效的情况下,配合床边连续性血液净化治疗,取得较好疗效。

Objective To investigate the clinical application of CBP in acute severe phosgene poisoning with MODS. Methods One patient with severe acute phosgene poisoning and MODS was treated with CBP. Clinical data including clinical symptoms, signs, arterial blood and radiographic changes will be collected to retrospectively analyze the efficacy before and after treatment. Results The combined therapy was shown invalid to the patient when the patient was in mechanical ventilation and glucocorticoid-based therapy. After CBP treatment was adopted, clinical symptoms and signs was improved rapidly, arterial blood gases returned to normal and lung imaging changes markedly improved. Conclusion For the patients who was suffering from severe acute phosgene poisoning combined multiple organ dysfunction syndrome, beside continuous blood purification therapy could be used in the case of ineffective treatment after conventional treatment to achieve good effect.
