医疗失效模式与效应分析在降低胃管滑脱率中的应用

Application of Healthcare Failure Mode and Effect Analysis in decreasing the incidence of gastric tube slippage

目的探讨医疗失效模式与效应分析在降低胃管滑脱率中的应用效果。方法根据医疗失效模式与效应分析工作流程,确定主题,组建团队,制订流程图,分析各流程步骤中的潜在失效模式、失效原因及失效结果,进行严重度、可能性、危机值分析,根据分析结果制订相应改进措施并落实。结果胃管脱出率由3.52%下降至0.45%(P<0.05)。结论应用医疗失效模式与效应分析有助于细化胃管护理流程,降低胃管脱出率。

Objective To explore the effects of healthcare failure mode and effect analysis （HFMEA） in decreasing the incidence of gastric tube slippage. Methods According to the process of HFMEA,the methods included determining the subject,establishing team,formulating the flow chart,analyzing the possible mode, reasons and results of failure and severity, possibility and crisis value, formulating and implementing the improvement measures. Result The incidence of gastric tube slippage decreased from 3.52% to 0.45 % （ P 〈 0. 05 ）. Conclusion Utilization of HFMEA is effective on refining gastric tube nursing process, thus to decrease the rate of gastric tube slippage.
