蜂蜜敷料治疗糖尿病慢性伤口的效果及安全性评价

Effects and Safty of Honey Dressings in the Treatment of Diabetes Chronic Wound

目的评价蜂蜜敷料治疗糖尿病慢性伤口的效果及安全性。方法选择2010年1月—2011年12月在三峡大学第一临床医学院外科门诊就诊的糖尿病慢性伤口患者99例,采用信封抽取的随机方法分成A、B、C 3组,每组33例。A组采用蜂蜜敷料,B组采用功能性敷料,C组采用活力碘敷料换药。观察3组伤口细菌清除时间及治疗时间、二次创伤积分及A组患者治疗前后血糖水平的变化。结果 3组伤口细菌清除时间、治疗时间和二次创伤积分比较,差异均有统计学意义(P<0.05);其中A组和B组伤口细菌清除时间、治疗时间和二次创伤积分较C组降低,差异均有统计学意义(P<0.05);A组伤口细菌清除时间和治疗时间较B组降低,差异均有统计学意义(P<0.05);A组与B组二次创伤积分比较,差异无统计学意义(P>0.05)。A组患者3次换药前后血糖水平比较,差异均无统计学意义(P>0.05)。结论蜂蜜敷料处理糖尿病慢性伤口能快速清除感染和坏死组织,促进伤口愈合,没有出现敷料二次损伤和组织吸收问题,是一种安全有效的治疗方法。

Objective To evaluate the effects and safty of honey dressings in the treatment of diabetes chronic wound. Methods A total of 99 patients with diabetes chronic wound visited outpatient department of surgery in the first clinical medical college of three gorges university between Jan 2010 and Dec 2011 were randomly and evenly divided into A, B and C groups which were treated with home - made honey dressings, functional dressings and conventional dressings respectively. The 3 groupshacterial clearance rate, treatment time and dressing secondary damage points were recorded. Blood sugar of group A was monitored before and after the treatment. Results Among 3 groups, the time of bacterial clearance and treatment, dressing sec- ondary damage points were significantly different （P 〈 0. 05） ; The time of bacterial clearance and treatment, dressing secondary damage points of group A and B were decreased compared with group C （P 〈 0.05） ; The time of bacterial clearance and treat-ment of group A were significantly shorter than those of group B （P 〈 0. 05 ） ; Dressing secondary damage points of group A and B were not significantly different. （ P 〉 0.05 ） . No significant differences were observed in blood sugar levels of group A before and after the honey dressing treatment （P 〉 0.05） . Conclusion Honey dressing can rapidly remove the infected and necrotic tissue, promote wound healing, and showes no signs of dressing secondary damage and tisse absorbtion problem. Honey dressings is a safe and effective way for treating diabetes chronic wound.
