摘要
单纯呼吸道蒸汽烧伤和呼吸道蒸汽烧伤复合体表烧伤肾脏病变的性质基本一致,表现为部分肾小球系膜区增宽,毛细血管袢有不同程度的缺血。超微结构显示血管球细胞呈现肥大、增生和肿胀两方面的变化。肾小管呈现空泡变、坏死及管型形成。空泡变和坏死主要累及近曲小管,管型则多见于远曲个管和髓质小管。但它们在发生频数和程度上复合组与单呼组有所差异。文中对病变的发生机理进行了简要讨论。血生化检查结果提示,本实验材料肾功能有一定程度的损害。从形态学角度来看,这种损害考虑系由肾小管坏死和肾小球缺血综合作用所致,前者可能起着更为重要的作用。
The pathological features of the kidneys are rather consistent in both simple respiratory burn and combined (both respiratory and body surface) burn. In part of glomeruli, capillary loops reveal ischemia and the mesangial areas become widened. Ultrastructurally, glomerular cells exhibit hypertrophy and hyperplasia on the one hand, and ceiluiar swelling on the other. The renal tubules show varying degrees of vacuolar degeneration, necrosis and cast formation. Vacuolar degeneration and necrosis affect more frequently the proximal convoluted tubules. Hyaline casts are frequently seen in the lumina of distal convoluted tubules, Henle's loops and collecting tubules. There are some differences in frequency of occurrence and degree of the lesions in both groups. The etiological factors and mechanism of the lesions are briefly discussed.Laboratory data indicate that there is an impairment of renal function in certain degree. From the morphological point of view, the impairment of renal function might be due to the composite effects of tubular necrosis and glomerular ischemia.
