显微根管治疗技术处理复杂根管病例的临床疗效分析

Evaluation of the Clinical Effect of Microscopic Root Canal Therapy Technique in the Management of Complicated Root Canal Cases

目的评价牙科手术显微镜(Dental operating microscope,DOM)联合超声器械处理复杂根管病例的临床效果,为推广应用显微根管治疗技术提供指导。方法收集2009年1月至2012年10月期间收治的375例显微根管治疗病例,共586个根管的数据信息,按根管钙化、根管变异遗漏、根管治疗不完善、器械分离、根管侧穿等原因分类进行统计分析,总结各种治疗类型的临床效果。结果 323颗患牙成功完成治疗,成功率86.13%;521个根管成功完成治疗,成功率88.91%。疏通钙化根管、探查遗漏根管、根管再治疗、取分离器械、修补侧穿的成功率分别为85.71%、96.03%、88.52%、82.22%、90.32%。结论显微根管治疗技术处理复杂根管的成功率高,可使许多不同类型的复杂根管病例得到理想的治疗效果。

Objective To evaluate the clinical effect of dental operating microscope in the management of complicated root canal cases combined with ultrasonic instruments, to provide guidance for promotion and application of microscopic root canal therapy technique. Methods Summarize the data information of 586 complicated root canals of 375 teeth which were treated with microscopic root canal therapy technique between January 2009 and October 2012. Results 323 teeth were successful managed with a success rate of 86.13%; 521 canals were successfully completed with a success rate of 88.91%. The success rates of different categories of treatments were ： 85.71% for Dredging calcified root canal, 96.03% for exploration of missed canals, 88.52% for root canal retreatment, 82.22% for taking separation equipment, and 90.32% for repair root canal perforation. Conclusion Microscopic root canal therapy always have high success rate in treatment of complicated root canals, can make many different types of complicated root canal cases obtained ideal treatment effect.
