摘要
目的分析规范化社区综合管理对社区高血压患者血压控制效果,为进一步加强社区高血压的防治提供相关依据。方法将上海某社区2009~2012年度2196例原发性高血压患者按照《上海市社区高血压防治工作指南》进行规范化社区管理,连续管理3年后,将管理前后结果进行比较和分析。结果①血压分级情况:管理3年后,血压分级情况逐渐好转,前后比较差异有统计学意义(P〈0.05)。②管理前后血压水平比较收缩压和舒张压均有所下降,差异有统计学意义(P〈0.01)。③管理3个年度的血压控制情况,优良和尚可的人数逐渐增加,不良的人数在下降,前后比较差异有统计学意义(P〈0.01)。④服药情况,管理前服药率为76.4%,管理后为90.3%,差异有统计学意义(P〈0.01)。结论通过对社区高血压患者的规范化管理,患者的血压分级逐渐好转,血压水平有所下降,血压控制情况好转,服药率上升。
ObjectiveTo analyze the effect of standardized community-based comprehensive management on blood pressure control in hypertension patients, and to provide basis for further prevention and treatment of hypertension in community.MethodsWe administrated 2196 patients with primary hypertension,according to the "Shanghai guidelines for community-based prevention and treatment of hypertension", in a subdistrict in Shanghai. After 3 years standardized management, we compared the blood pressure control before and after management.ResultsAfter 3 years standardized management, the classification of blood pressure turned better, and the classification was statistically different before and after management(P0.05). Both systolic blood pressure and diastolic blood pressure were declined, and both of the changes were significant(P0.01). The number of patients with excellent and ordinary control of blood pressure increased gradually, and the number of poor control declined, and the change was significant(P0.01). The medication rate was 76.4% before management, and it rose to 90.3% after the management, and the difference was significant(P0.01).Conclusion After standardized management, the blood pressure declines in community-based hypertension patients. The classification of blood pressure turns better, so does the control of blood pressure. The regular medication rate in hypertension patients rises.
