MRI诊断乳腺X线非对称性致密影病变的临床价值

Chinical Value of MRI Diagnosis Breast X-ray Asymmetrical Dense Shade Lesions

目的：探讨MRI诊断乳腺X线非对称性致密影病变中良、恶性疾病的诊断价值。方法：选取2010年11月-2013年2月的80例乳腺X线非对称性致密影病变患者为研究对象,病变无肿块、钙化、结构扭曲等,全部采用MRI扫描,观察病灶的形态、分布、动态增强、时间-曲线和周围影像学改变。结果：肿块型强化15例,非肿块型强化65例；病灶强化均匀9例,不均匀71例；其中伴有多发脓肿、壁呈均匀或不均匀强化3例；病变乳内血管异常改变4例,皮肤表面增厚4例,皮下水肿2例,淋巴结增大1例；时间-曲线Ⅰ型39例,Ⅱ型24例,Ⅲ型17例。乳腺恶性病变31例（其中导管原位癌7例,浸润性导管癌5例,小叶原位癌6例,浸润性小叶癌2例,乳腺血管肉瘤3例,乳腺导管癌合并乳腺小叶癌8例）；良性病变49例（其中乳腺增生病6例,硬化性腺病10例,慢性炎症7例,肉芽肿性乳腺炎5例,浆细胞性乳腺炎6例,囊肿伴感染4例,纤维腺瘤3例）。结论：MRI能发现乳腺X线非对称性致密影病变的良、恶性病变,特别是对于乳腺癌的早期诊断有重要价值。

Objective：To study the MRI in the diagnosis of breast X-ray asymmetrical dense shadow the diagnostic value of benign and malignant lesions of the disease.Methods：Select 2010-February 2013 in November of 80 cases of mammary X-ray asymmetry density shadow lesion patients as the research object,lesions without mass,calcification,structure distortion and so on,all using MRI scans,observe morphology and distribution of lesions,dynamic enhancement,time-around the curve and the imaging changes.Results：The tumor type 15 cases,65 cases non mass reinforcement;Uneven lesions improved uniform 9 cases,71 cases;Which accompanied by multiple abscess,wall is uniform or non-uniform strengthen 3 cases;Breast lesions abnormal changes of blood vessels in 4 cases,4 cases were thickening of the skin,subcutaneous edema in 2 cases,lymph node enlargement in 1 case;Time-curveⅠ39 cases,Ⅱtype 24 cases,17 casesⅢtype.Malignant breast lesions in 31 cases （ductal carcinoma in situ,7 cases,the infiltrating ductal carcinoma in 5 cases,6 cases of lobular carcinoma in situ and invasive lobular carcinoma 2 cases,breast angiosarcoma （3 cases）,mammary ductal carcinoma combined lobular carcinoma （8 cases）;Benign lesions in 49 cases （6 cases of mammary gland hyperplasia,sclerosing adenosis 10 cases,7 cases of chronic inflammation,5 cases of granulomatous mastitis,plasma cell mastitis 6 cases,cyst associated with infection,4 cases of fibroadenoma 3 cases）.Conclusion：MRI can find breast X-ray asymmetrical dense shade lesions of benign and malignant lesions,especially for the early diagnosis of breast cancer has important value.
