摘要 目的探讨凋亡抑制蛋白Livin(活素)和促凋亡因子Smac(第二个线粒体衍生的半胱氨酸蛋白酶激活剂)在日光性角化病(AK)中的表达及意义。方法应用免疫组化SP法检测76例AK组织中Livin和Smac的表达,分析二者表达的相关性。结果 Livin在AK组织中的阳性表达率为55.26%,在正常对照组为3.33%,二者比较差异有统计学意义(P<0.001)。Livin在不同病理亚型AK(肥厚型、萎缩型、苔藓样型和鲍恩样型)中的阳性表达率分别为52.00%,52.94%,45.45%和72.73%,各组之间比较差异无统计学意义。Smac在AK组织中的阳性表达率为56.58%,在正常对照组为100.00%,二者比较差异有统计学意义(P<0.001)。Smac在不同病理亚型AK(肥厚型、萎缩型、苔藓样型和鲍恩样型)中的阳性表达率分别为56.00%,64.71%,63.64%和45.45%,各组之间比较差异无统计学意义。Livin和Smac在AK中的表达呈负相关(r=-0.468,P<0.01)。结论肥厚型、萎缩型、苔藓样型和鲍恩样型AK中Livin和Smac的表达与病理亚型无关,Livin的过表达和Smac的低表达可能在AK的发生、发展中起一定作用。 Objective To investigate the expression and significance of Livin and Smac protein in actinic keratosis (AK). Methods Immunohistochemical staining technique was used to detect the expression levels of Livin and Smac protein in 76 cases of AK and 30 normal controls. The relationship between the expression of Livin and Smac protein in AK was analyzed. Results The positive rates for Livin expression were significantly higher in AK( 55.26% ) than in normal skin(3.33% ) (P 〈0. 001 ). The positive rates for Livin expression in hypertrophic variant, atrophic variant, lichenoid variant and bowenoid variant were 52.00%, 52.94%, 45.45% and 72.73% , respectively. There were no significant differences in the positive rate of Livin expres- sion among different variants of AK. The positive rate for Smac expression was significantly lower in AK (56.58%) than in normal skin( 100.00% ) (P 〈0.001 ). The positive rates for Smac expression in hyper- trophic variant ,atrophic variant, lichenoid variant and bowenoid variant were 56.00%, 64.71%, 63.64% and 45.45%, respectively. There were no significant differences in the positive rate of Smac expression among dif- ferent variants of AK. Livin expression negatively correlated with Smac expression in AK( r = - 0. 468, P 〈 0.01 ). Conclusion The expression levels of Livin and Smac in AK are not associated with variants. The overexpression of Livin and low expression of Smac may be associated with the onset and the development of AK.
