外固定支架结合有限内固定治疗Ⅱ、Ⅲ型Pilon骨折的临床观察

Treatment of type Ⅱ,Ⅲ Pilon fracture by external fixator combined with limited internal fixation

目的探讨外固定支架结合有限内固定术治疗Ⅱ、Ⅲ型Pilon骨折的临床疗效。方法选择2008年1月到2012年12月本院收治的52例Pilon骨折患者作为观察对象,在征得患者及家属同意并签署知情同意书后,随机将患者分为对照组(27例)和观察组(25例),其中对照组患者行切开复位内固定治疗方案,而观察组患者行外固定支架结合有限内固定术治疗方案,对比两组患者治疗效果及不良反应发生率。结果观察组治疗优良率96.00%,对照组治疗优良率77.78%,两组比较差异显著(P<0.05);观察组不良反应发生率8%,对照组不良反应发生率11.11%,两组比较无显著性差异(P>0.05);观察组平均住院时间(15.87±4.15)天,明显低于对照组平均住院时间(20.34±6.42)天,两组比较差异具有统计学意义(P<0.05)。结论对Ⅱ、Ⅲ型Pilon骨折患者行外固定支架结合有限内固定术疗效令人满意,同时该治疗方案相对安全,术后不良反应发生率低。

Objective To observe the clinical effect of external fixator combined with limited internal fixation in the treatment of type Ⅱ, Ⅲ Pilon fracture. Methods A total of 52 patients with Pilon fracture from Jan 2008 to Dec 2012 were divided into two groups. 27 cases treated with open reduction and internal fixation were taken as the control group, the other 25 cases treated with external fixator combined with limited internal fixation were taken as the observation group. The clinical effect and the incidence of adverse reactions of two groups were observed and compared. Results The excellent and good rate was 96% in the observation group, and 77.78% in the control group, the difference between was significant （P〈 0.05）.The adverse reaction in observation group was 8%, 11.11% in the conrrol group, the two groups had no significant difference （P〉0.05）.The aver- age hospitalization time was （15.87±4.15）d in the observation group, significantly lower than that in the control group which was （20.34±6.42）d （P〈 0.05）. Conclusion The clinical effect external fixator combined with limited internal fixation in the treatment of type Ⅱ, Ⅲ Pilon fracture is satis- fied and safety, and the incidence of postoperative adverse reaction is low.
