温肾活血方治疗寒凝血瘀型子宫内膜异位症不孕患者的临床观察

Clinical Observation on “Wenshen Huoxue Decoction”in Treatment of Endometriosis Associated Infertility of Cold Stagnation and Blood Stasis Type

目的:观察温肾活血方治疗寒凝血瘀型子宫内膜异位症(EMs)不孕患者的疗效。方法:将60例EMs不孕患者随机分成治疗组和对照组,每组30例。治疗组采用温肾活血方治疗,对照组采用少腹逐瘀汤治疗,治疗周期为12个月。比较两组的临床疗效,观察两组患者的临床症状积分、基础体温(BBT)、排卵情况及血清促卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E2)、睾酮(T)、泌乳素(PRL)水平的变化。结果:治疗组妊娠14例、总有效率为90.00%,对照组妊娠6例、总有效率为76.67%,两组比较差异有统计学意义(P<0.05)。治疗后,两组患者的临床症状积分均显著降低(P<0.01),且治疗组患者的积分低于对照组(P<0.05);治疗组患者的BBT情况明显改善(P<0.01),且治疗组患者的BBT情况优于对照组(P<0.05);治疗组患者的卵泡发育情况明显改善(P<0.01),且治疗组患者的卵泡发育情况优于对照组(P<0.05)。治疗后,治疗组患者的FSH、FSH/LH、PRL水平明显降低(P<0.01),且治疗组患者的LH水平高于对照组(P<0.05),FSH/LH水平低于对照组(P<0.05)。结论:温肾活血方可改善寒凝血瘀型子宫内膜异位症不孕患者的卵巢功能,提高妊娠率。

Objective：To observe the effect of ＂Wenshen Huoxue Decoction ＂ on endometriosis（ EMs） associated infertility of cold stagnation and blood stasis syndrome.Methods：A total of 60 patients with EMs associated infertility were randomized into treatment group in which 30 cases were treated by＂Wenshen Huoxue Decoction＂,and control group in which 30 cases were treated by＂Shaofu Zhuyu Decoction＂.The treatment course was 12 months.The clinical effect was evaluated,and the symptom scores,basal body temperature,ovulation and the levels of follicle stimulating hormone（ FSH）,luteinizing hormone（ LH）,estradiol（ E 2）,testosterone（ T） and prolactin（ PRL） were observed.Results：Fourteen cases were pregnant in treatment group,and six cases were pregnant in control group; the total effective rates of treatment group and control group were 90.00% and 76.67% respectively,with statistical difference between two groups（ P 0.05）.After treatment,the symptom scores of two groups were markedly decreased（ P 0.01）,and the scores of treatment group were lower than those of control group（ P 0.05）; the condition of BBT in treatment group was significantly improved（ P 0.01）,and the condition of BBT in treatment group was better than that in control group（ P 0.05）; the ovarian follicular development in treatment group was obviously improved（ P 0.01）,and the ovarian follicular development of treatment group was better than that of control group（ P 0.05）.After treatment,the levels of FSH,FSH / LH and PRL were remarkably decreased（ P 0.01）,and the level of LH of treatment group was higher than that of control group（ P 0.05）,the level of FSH / LH of treatment group was lower than that of control group（ P 0.05）.Conclusion：＂Wenshen Huoxue Decoction＂can improve the ovarian function of patients with EMs associated infertility of cold stagnation and blood stasis syndrome,and enhance the pregnancy rate.
