摘要
目的分析机采血小板献血反应发生的原因,总结预防其发生的措施,为采供血机构降低机采血小板献血反应发生率提供帮助。方法检索CNKI,万方医学数据库,维普数据库2001—2011年文献,检索关键词为“机采血小板”、“单采血小板”和“献血反应”,选取文献中有统计数据的相关文献,进行统计分析。结果统计机采血小板献血总人数为105695人次,献血反应发生率2.86%。献血反应程度轻度占85.94%,献血反应诱因主要为构橡酸盐反应和精神因素,分别占38.82%和20.20%。献血反应的发生在性别、献血次数、血液采集量、血液流速、抗凝剂比例、预防服钙和护理干预与否方面差异有统计学意义(P〈0.05),在不同年龄、体重、循环血量和采血机型方面差异无统计学意义(P〉0.05)。结论在严格执行《献血者健康检查要求》基础上,控制机采过程中的血液流速,预防性口服钙剂和通过护理干预等均可降低献血反应的发生。
Objective To analyze the causes of blood donation reaction by plateletpheresis, summarize the preventive measures and provide assistance for blood banks to lower blood donation reaction incidence rate. Methods First,the CK- NI,WANFANG Medicine Database and VIP Database are searched with key words of "Machine-collected platelets" ," sin- gle donor platelets" and "blood donation reactions". Then,the relevant documentations in years 2001 ~ 2011 containing statistical data are selected. Finally, a statistical analysis is made on the basis of these documents. Results Among a total number of 105 695 blood donors by machine-collected platelets,the blood reaction incident rate is 2.86%. Blood donation leading to mild effects accounts for 85.94%. The causes for the effects are attributed to citrate reactions and psychological factors, accounting for 38.82% and 20.20 % respectively. The incidence of blood donation reaction has statistical significance ( P 〈 0. 05 ) with regard to gender, age, weight, number of donation times, blood collection volume, blood flow speed, anticoagulant proportion, preventive uptake of calcium and nursing intervention. It has no statistical significance with regard to circulation blood volume and type of blood collection machines (P 〉 0. 05 ). Conclusion On the basis of strict compli- ance with the Requirement for Blood Donor's Health Examination, measures such as control of blood flow speed in course of machine collection,preventive uptake of calcium and nursing intervention can contribute to lower incidence of blood donation reaction.
