摘要
目的探讨微创经皮钢板骨桥接技术结合锁定加压板治疗胫骨开放性骨折的临床疗效。方法对2011年5月至2012年10月共24例胫骨开放性骨折、局部软组织条件差的患者,均采取一期清创缝合,二期行MIPPO技术结合LCP治疗,早期功能锻炼,24例均获随访,随访6-11个月,平均8个月。结果切口均一期愈合;骨折愈合时间3-11个月,平均5个月,无感染,无骨折延迟愈合以及不愈合等并发症。术后膝踝关节功能以Johner-Wruhs方法评价,优17例,良5例,可2例。结论MIPPO技术下LCP钢板治疗胫骨开放性骨折,创伤小,能减低对骨折及软组织血运的干扰,提高骨折愈合率,是治疗胫骨开放性骨折的理想的方法。
Objective To explore the therapeutic effectiveness of MIPPO technique and locking compres- sive plate for treating open tibial fractures. Methods From May 2011 to October 2012, 24 patients of open tibial fractures and serious soft tissue injuries were treated by debridement and suturing firstly, then secondary operation by MIPPO Technique and locking compressive plate, and early functional exercise. All the patients were followed up for 6-11 months, with an average of 8 months. Results Operative incision of the 24 patients showed primary healing. Time of bone union was 3-11 months, with an average of 5 months. No case sustained infection, bone nonunion or late healing. According to the criteria of Johner-Wruhs score, there were 17 cases assessed as being excellent, 5 cases as good and 2 cases as acceptable. Conclusion Treatment of open tibial fractures with MIPPO technique and locking compressive plate has less invasion and higher union rate, which is an ideal method.
