摘要
目的 评价基因重组乙型肝炎 (乙肝 )疫苗对人群的免疫效果。方法 对 195名新生儿、191名中学生及 2 19名不同HBV感染者接种疫苗 ,进行效果观察。并比较重组酵母来源和血源来源的 2种不同检测乙肝表面抗体的放射免疫分析方法试剂。结果 新生儿免疫后 ,抗 HBs阳转率均达到 85 %以上 ,但中华地鼠卵巢细胞基因重组乙肝疫苗不论是在抗体阳转率还是在抗体滴度上效果都优于酵母乙肝疫苗。中学生免疫后 ,抗 HBs阳转率达 70 %以上 ,免疫后 2年时 10 μg 5 μg 5 μg剂量组的免疫效果要优于 5 μg 5 μg 5 μg组。对HBV不同感染者免疫后 ,可使 5 1 14%的感染者抗 HBs阳转或抗 HBs滴度上升。重组酵母来源试剂检测抗 HBs的阳转率 (81 42 % )比血源来源试剂检测(73 33% )可提高近 10 %。结论 基因重组乙肝疫苗的免疫效果良好 ,对青少年接种疫苗可采用接种前不检测HBV抗体策略 ,以酵母乙肝疫苗 10 μg 5 μg 5 μg剂量为宜。在监测人群免疫后抗 HBs时 ,应选用相应抗原制备的试剂检测相应抗体 。
Objective To evaluate the immune effect of different doses and different sort of gene recombinant hepatitis B vaccine in various populations. Methods Using the method of serum epidemiology,195 newborns from HBsAg negative mothers were divided into 3 groups and treated with different doses of HBV vaccine,191serum HBsAg negative high school students were divided into 2 groups and treated with 2 different HBV doses,they were vaccinated with hepatitis B vaccine by 0,1,and 6 month schedule and observed for 1 2 years to compare the immune effect In addition,two kinds of anti HBs RIA reagents prepared from blood or recombinant yeast were compared for the assay result. Results After the newborns were vaccinated with gene recombinant hepatitis B vaccine,their anti HBs positive conversion rate was above 85%,but the effects of Chinese hamster ovary cell recombinant hepatitis B vaccines (CHO) was better than that yeast recombinant hepatitis B vaccines both in antibody positive conversion rate and antibody titer The anti HBs positive conversion rate in the high school students reached 72 63% 75 00% after treated with hepatitis B vaccine and the immune effect in the 10,5,5 μg group was better than the 5,5,5 μg group after 2 years After vaccination of HBV infected subjects,about 68 49% turned to be anti HBs positive or their anti HBs titer increased The positive rate detected by the recombinant yeast originated RIA reagents was higher than that detected by blood originated reagent (81 42% vs 73 33%). Conclusions The immune effect of gene recombinant hepatitis B vaccine was confirmed in different populations There is no need to measure HBV M before vaccination in the high school students,and yeast recombinant vaccine can be used with doses of 10,5 and 5 μg When monitoring the anti HBs level in populations after vaccination,the reagents prepared corresponding antigens sould be used for detecting the homologous antibody to increase the sensitivity of detection
