小剂量雌激素治疗Ⅰ型原发性骨质疏松症的初步效果

THE EFFECT OF SMALL-DOSE ESTROGEN IN THE TREATMENT OF TYPE Ⅰ PRIMARY OSTEOPOROSIS

①目的 探讨小剂量雌激素治疗Ⅰ型原发性骨质疏松症的方法和效果。②方法 选择Ⅰ型原发性骨质疏松症病人 96例 ,随机分为实验组 (48例 )和对照组 (48例 ) ,对照组病人按常规服用结合型雌激素 0 .6 2 5mg/d ,连用 2 1d(其中后 10d加用黄体酮 2 .5 0 0mg/d) ,停药 10d为 1个疗程。实验组隔日服用结合型雌激素 0 .6 2 5mg ,其他同对照组。两组均治疗 4个疗程 ,治疗前后分别进行血碱性磷酸酶、尿钙 /肌酐比值和骨密度测定 ,并进行临床疗效评定。③结果 两组的有效率分别为 95 .8%和 91.7% ,差异无显著性 (χ2 =2 .13,P >0 .0 5 )。血碱性磷酸酶两组治疗前后差异无显著性 (t =0 .0 2 ,0 .17,P >0 .0 5 ) ;尿钙 /肌酐比值两组治疗前后均有明显降低 (t =2 .2 0 ,2 .11,P <0 .0 5 ) ;骨密度值两组治疗前后差异无显著性 (t=0 .0 0 0 0 0 2 ,P >0 .0 5 )。④结论 小剂量与常规剂量雌激素治疗Ⅰ型原发性骨质疏松症病人的效果相同 ,但可以减轻副作用 。

Objective To study the methods and effects of small-dose estrogen in the treatment of type Ⅰ primary osteoporosis.\ Methods 96 patients with type Ⅰ primary osteoporosis were randomly divided into experimental group(n=48) and control group(n=48) . The hapt-estrin 0.625mg per day orally for control group for 21 days with the addition of progesterone 2.5mg per day in the later 10 days, then the drugs discarded for ten days to be served as one course. The hapt-estrin 0.625mg every other day for experimental group , the others were the same as the control . Each group received four courses of the treatment. The ratio of uric calcium and creatinine, AKP, bone density were assayed before and after the treatment to evaluate the efficacy.\ Results The effect rate was 95.8% and 91.7%, respectively (χ 2=2.13,P>0.05). The AKP didn't change distinctively before and after treatment in both groups(t=0.02,0.17,P>0.05). The ratio of uric calcium and creatinine decreased remarkably before and after the treatment(t=2.20, 2.11,P< 0.05). The bone density had no obvious difference before and after the treatment (t= 0.000 002 ,P>0.05). \ Conclusion The effect was the same with small-dose or routine-dose estrogen in the treatment of type Ⅰ primary osteoporosis, but small-dose estrogen can reduce the side effects.
