乳腺癌保乳综合治疗

Breast conserving surgery for breast cancer patients.

目的 探讨乳腺癌保乳综合治疗的原则及近期疗效。方法 82例各期乳腺癌病例施保乳手术 (局部乳腺切除 +腋淋巴结清扫术 )、术中或术后瘤床插植近距放疗 +化疗。手术后 2～ 4周再接受全乳腺外照射剂量为 5 0Gy。激素受体阳性的病人均接受三苯氧胺治疗 ,随访时间中位数为 4 2个月。结果 82例全部存活 ,无一例出现局部复发。 1例出现骨转移。双乳对称 ,外形美观。其中 1例为双侧乳癌 ,两侧病理类型不同 ,均做保乳手术。结论 早期乳腺癌病人接受保乳综合治疗可以取得满意的临床效果 ,可成为早期乳腺癌的首选治疗方法。

Objective To evaluate the treatment principle and the preliminary outcome of breast cancer treasted with breat-conserving surgery plus radical breast irradiation and chemotherapy.Methods 82 breast cancer patients were treated with tumorectomy or quadrant-ectomy and axillary nodes dissection followed by intro-or after operation btachytheray of the tumor bed and total breast irradiation+chemtherapy.Median follow-up was 42 months (Minimum 10 months;maximun,94 months).Results All patients were alive without local recurrence.Metastasis of bone was found in one patient.There was no late side effect in the breast and lung.Good cosmesis was noted in all of the patients except one.One case was bilateral breast cancer.Both of sides were received breast-conserving surgery.Conclusion Breast-conservng surgery and radical radiation therapy is effective for patients with early stage of cacinoma of breasts. It is more difficult to develop because of the domestic new market economy system, the occurrence of buyer market, and the deep system reform. The good condition included twenty years' economy outcome, the experience of east reform, and the high studying point of superiority. So, forestry construction is the subject of west development. From theory, the beautiful landscape and flourished economy is depending on each other. From policy, the forestry construction is emphasized strongly. From reality, the west area is fit to planting forest. And the ecological and economy construction should proceed at the same pace, from our history experience.
