延续性护理对狼疮肾炎患者肾损伤相关标记物的影响

Effect of continuous nursing on neutrophil gelatinase-associated lipocalin in patients with lupus nephritis

目的探讨延续性护理对狼疮肾炎患者血肌酐(SCr)、尿素氮(BUN)及血/尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)的影响。方法选取武汉市第一医院肾内科狼疮肾炎患者60例,随机分为对照组和观察组,各30例,对照组于出院时给予常规护理指导,观察组患者给予延续性护理干预,于患者出院时和出院6个月后检测血液中的SCr、BUN及血/尿NGAL水平,并评估患者的满意度。结果患者出院6个月后复查结果显示,观察组患者的SCr、BUN及血/尿NGAL水平较出院时无明显上升(P>0.05),且明显低于对照组患者(P<0.05),观察组患者对护理满意的比例(93.3%)明显高于对照组(60.0%),差异有统计学意义(P<0.05)。结论狼疮肾炎患者在症状稳定出院后给予延续性护理干预,能够控制患者SCr、BUN及血/尿NGAL的水平,进一步稳定患者病情,提高患者的护理满意度,值得临床上进一步推广。

Objective To investigate the influence of continuous nursing on serum creatinine(SCr)and blood urea nitrogen(BUN),serum and urinary neutrophil gelatinase-associated lipocalin(NGAL)level in patients with lupus nephritis(LN).Methods Sixty cases of LN were randomly divided into 2 groups:control group(routine nursing)and observation group(continuous nursing).Levels of SCr and BUN in serum,serum and urinary NGAL were measured by ELISA at discharge and 6 months after hospital discharge.Results At 6th month after discharge,serum SCr and BUN,serum and urinary NGAL had no significant elevation in observation group as compared with those at discharge,and significantly reduced in observation group as compared with those in control group(P<0.05).The satisfaction in observation group was significantly higher than that in control group(93.3% vs.60.0%,P<0.05).Urinary NGAL was closely correlated with LN.Conclusions Continuous nursing intervention can control SCr,BUN and NGAL levels in patients with LN after the symptoms are stable and discharged.Continuous nursing can slow the progression of the disease,and improve the degree of patient's satisfaction,which is worth further promotion in the clinical practice.
