摘要
目的探讨早期磨削术联合重组牛碱性成纤维细胞生长因子(recombinant bovine basic fibroblast growth factor, rb-bFGF)凝胶治疗儿童深Ⅱ度烧烫伤的临床疗效。方法对2017年3月至2018年2月福建医科大学附属闽东医院烧伤整形外科收治的50例深Ⅱ度烧烫伤患儿创面早期采用磨削术联合rb-bFGF凝胶治疗,同时予以抗休克、抗感染、营养支持等全身综合治疗,观察治疗效果。结果 50例患儿中除1例患儿因治疗2周后创面无明显愈合倾向而选择植皮外,其余患儿创面均一期愈合,愈合时间为12~20 d,且愈后皮肤除留有少许色素沉着或脱失外,均无明显增生性瘢痕形成。结论早期磨削术联合rb-bFGF凝胶治疗儿童深Ⅱ度烧烫伤创面,可有效促进创面愈合,降低增生性瘢痕的形成机率,疗效显著,且操作简便,值得临床推广应用。
Objective To investigate the clinical efficacy of abrasion at early stage combined with rb-bFGF gel in the treatment of deep second degree burns and scalds in children. Methods Fifty children patients with deep second degree burns and scalds, admitted into the Department of Burns and Plastic Surgery, Mindong Hospital of Ningde City from March 2017 to February 2018, were treated with abrasion at early stage combined with rb-BFGF gel. Meanwhile, other systemic comprehensive treatments were given, such as anti-shock, anti-infection, nutritional support. The treatment effects were observed. Results Except one patient who accepted skin grafting due to no obvious sign of wound healing after receiving the studied treatment approach for two weeks, the rest 49 children patients all realized primary healing, the healing time was 12-20 days and no obvious hypertrophic scar was found on the healed skin but with mild pigmentation or depigmentation. Conclusion The treatment approach of abrasion at early stage combined with rb-bFGF gel in the treatment of deep second degree burns and scalds in children can effectively promote wound healing and reduce the formation of hypertrophic scars. Given the remarkable curative effect, easy-and-simple operation and lower cost, this approach deserves to be promoted in clinical practice.
