急诊腹腔镜治疗异位妊娠58例临床分析

Clinical analysis of treatment of ectopic pregnancy in emergency by laparoscopy in 58 cases.

目的 探讨急诊状态下腹腔镜治疗异位妊娠的可行性、安全性及治疗效果。方法 5 8例患者均在入院后 2h内完成术前准备 ,在气管插管全身麻醉下采用改良的三孔法置入腹腔镜器械 ,行输卵管切除术 4 9例、宫角切除术 2例、输卵管开窗术 7例。结果 所有手术均在腹腔镜下顺利完成 ,平均手术时间 4 2min ,除 2例出现脐部切口脂肪液化和 1例急性肾功能衰竭外 ,余无并发症出现。结论 急诊腹腔镜治疗异位妊娠 。

Objective To discuss the safety,effect and availability of the treatment of ectopic pregnancy with laparoscopy under the emergent condition.Methods The preparation for the operation in 58 ectopic pregancies had been finished within 2h after being admitted.The instruments were installed by three foramina methods after general anestheasia.Among them,49 cases had been operated with salpingectomy,2 cases with magulactomy and 7 cases with salpingotomy.Results All operations have been smoothly finished with laparoscopy without complications except 1 case with acute kidney function failure and 2 cases with liquefied fat around umbilicus.The average operation duration was 42 minutes.Conclusion Treating the ectopic pregnancy with laparoscopy under the emergent condition can be suggested as the regular operation.
