摘要
目的探讨细胞色素P450(CYP1A1)Msp I位点多态性与口腔癌易感性的关系。方法检索Pubmed、CNKI等数据库中有关CYP1A1 Msp I多态性与口腔癌易感性关系的文献,两名评价员独立筛选评价文献,以OR值和95%的可信区间(95%CI)为效应指标,应用STATA 11.0软件进行Meta分析,并对发表偏倚进行检验。结果纳入14个病例对照研究,包括1 914例口腔癌患者和2 610例健康对照,Meta分析结果显示,总人群中,m1/m1基因型的人群患口腔癌的风险低于m2/m2基因型(OR=0.43,95%CI:0.28~0.68)和m1/m2+m2/m2基因型(OR=0.66,95%CI:0.50~0.88),m1/m1+m1/m2基因型人群患口腔癌的风险低于m2/m2基因型(OR=0.47,95%CI:0.33~0.67)。种族的亚组分析中,亚洲人群的CYP1A1基因多态性与口腔癌易感性关系与总人群一致。对照组人群来源的亚组分析中,来自医院的m2/m2基因型人群会增加口腔癌的易感性(m1/m1 vs m2/m2,OR=0.40,95%CI:0.23~0.70;m1/m1+m1/m2 vs m2/m2,OR=0.42,95%CI:0.28~0.62),而m1/m1基因型的普通人群会降低口腔癌的易感性(m1/m1 vs m1/m2+m2/m2,OR=0.37,95%CI:0.21~0.66)。结论 CYP1A1多态性与亚洲人口腔癌易感性相关,携带m1等位基因的个体患口腔癌的风险可能会降低。
Objective To evaluate the association between CYP1A1 polymorphisms and oral cancer. Methods Relevant literatures were fully searched in electronic database such as Pubmed,CNKI,VIP,Science Direct. Two reviewers searched and evaluated the related studies independently. Statistical analyses were carried out using STATA 11. 0 software with calculating the pooled odds ratio( OR) with the corresponding 95 % confidence intervals( 95% CI) and detecting publication bias. Results Fourteen case-control studies including 1914 patients with oral cancer and 2610 healthy controls were selected. Of the total population,the pooled OR values of m1 / m1 compared with m2 / m2 and m1 / m2 + m2 / m2 were 0. 43( OR = 0. 43,95% CI: 0. 28 ~ 0. 68) and 0. 66( OR = 0. 66,95% CI: 0. 50 ~ 0. 88) respectively,and the pooled OR value of m1 / m1 + m1 / m2 compared with m2 / m2 was 0. 47( OR = 0. 47,95% CI: 0. 33 ~ 0. 67). Subgroup analysis of race,CYP1A1 polymorphisms associated with oral cancer susceptibility in Asian populations were consistent with the total population. Subgroup analysis of source control population,m2 / m2 genotype population from the hospital increased oral cancer susceptibility( m1 / m1 vs m2 / m2,OR = 0. 40,95% CI: 0. 23 ~ 0. 70; m1 / m1 + m1 / m2 vs m2 / m2,OR = 0. 42,95% CI: 0. 28 ~ 0. 62),while the general population of m1 / m1 reduced oral cancer susceptibility( m1 / m1 vs m1 / m2 + m2 / m2,OR = 0. 37,95% CI : 0. 21 ~ 0. 66). Conclusion CYP1A1 polymorphisms are associated with Asian oral cancer susceptibility,and individuals with m1 allele may reduce the possibility of oral cancer.
