2型糖尿病伴非酒精性脂肪肝患者血清脂联素水平的变化 被引量:4

摘要 目的:探讨2型糖尿病伴非酒精性脂肪肝患者血清脂联素水平变化及其临床意义。方法:241例观察对象分为81例2型糖尿病伴非酒精性脂肪肝组(T2DM+NAFLD)、80例单纯2型糖尿病(T2DM)和80例正常对照组(NC),测定腰臀围比值(WHR)、空腹血糖(FPG)、空腹胰岛素(FINS)、甘油三酯(TG)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、脂联素(APN)并对影响因素进行相关分析。结果:1T2DM+NAFLD组的BMI、WHR、TG、FPG、FINS、HOMA-IR、ALT、AST均显著高于T2DM组,APN低于T2DM组,差异具有统计学意义(P<0.05)。2TG、WHR是T2DM合并NAFLD的独立危险因素,APN是其保护因素。多元逐步回归分析表明TG、HOMA-IR和HDL-C是APN水平的主要影响因素。结论:低脂联素血症在2型糖尿病合并NAFLD的发生、发展中可能起重要作用。
