夹脊穴电针+复方当归注射液肌注+正骨去痛方外灸治疗强直性脊柱炎随机平行对照研究

Acupuncture and Compound Angelica Injection + Bone pain. Moxibustion Treatment of Ankylosing Spondylitis Random Parallel Control

[目的]观察夹脊穴电针＋复方当归注射液肌注＋正骨去痛方外灸治疗强直性脊柱炎疗效。[方法]使用随机平行对照方法,将114例住院患者按抽签法简单随机分为两组。对照组57例常规、综合治疗。治疗组57例选取病变脊柱上下关节夹脊穴,消毒后选用50mm规格的梅花针以80°倾角穿刺夹脊穴,待酸胀和麻木等触电类反射后留针;用针手法要注意轻柔,谨防误伤关节囊和皮下软组织;连接电针仪,按照2Hz规格电针治疗;复方当归注射液2mL,肌注;耐受性良好还可药物灸,正骨去痛方（黄芪20g,当归15g,白芍6g,细辛9g,红花20g,乳香5g,桂枝10g,没药5g,狗脊10g,续断15g,怀牛膝10g,鹿仙草5g）,制成粉剂;1次/d,每7d间隔1∽2d。连续治疗7d为1疗程。观测临床症状、主要体征（疼痛数值、枕墙距、指地距、Schober实验、胸廓活动度）、实验室指标（红细胞沉降率、C型反应性蛋白、免疫球蛋白、碱性磷酸酶）、不良反应。连续治疗5疗程,判定疗效。[结果]治疗组临床控制22例,显效19例,有效15例,无效1例,总有效率98.24%。对照组临床控制17例,显效23例,有效17例,无效10例,总有效率82.45%。治疗组疗效优于对照组（P〈0.05）。主要体征数据、实验室指标治疗组改善优于对照组（P〈0.05）。[结论]夹脊穴电针＋复方当归注射液肌注＋正骨去痛方外灸治疗强直性脊柱炎疗效显著,值得推广。

Observation of clinical symptoms, the main signs data, laboratory parameters, adverse reactions. 5 continuous treatment regimen to determine efficacy. [ Results ] The cure 22 cases, 19 cases were markedly effective in 15 cases, one case, the total efficiency of 98.24%. Cured 17 cases in the control group, 23 cases markedly effective in 17 cases, 10 eases, the total efficiency of 82.45%. Treatment group than the control group （ P 〈0.05 ） . The main signs data, laboratory parameters to improve the treatment group than the control group （ P 〈0.05 ） . [ Conclusion ] Acupuncture and compound angelica injection ＋ bone pain. Moxibustion treatment of ankylosing spondylitis random parallel control, the effect is significant, it is worth promoting.
