摘要
目的探讨应用Orthofix-微型外固定支架治疗第一掌骨基底部Bennett骨折的临床疗效。方法回顾性分析2008年1月至2013年1月该院收治的17例新鲜性Bennett骨折患者。其中男性14例,女性3例,均采用Orthofix-微型外固定支架治疗。术后根据骨折愈合情况,记录拆除外固定架时间。术后3个月与6个月,采用上肢臂、肩、手功能调查量表(DASH)进行评分,随访终末期采用指总关节活动度(TAM)进行评价。结果17例患者均获得随访,手术时间为20~40rain,平均29.1min,术中出血量10~35mL,平均12.5mL。术后X线片所有患者均骨性愈合,其中16例获得解剖复位,1例骨折对线良好,术后3、6个月、终末期DASH评分比较,差异均有统计学意义(P〈0.05)。随访终末期,TAM评分优14例,良2例,差1例。随访期间未发生钉道感染、桡神经浅支损伤等并发症。结论采用Orthofix-微型外固定支架治疗新鲜性Bennett骨折效果可靠、操作简单,具有一定的推广价值。
Objective To explore the application of Orthofix-mini external fixation for the treatment of clinical first basal metacarpal Bennett fracture. Methods A retrospective analysis of 17 cases of fresh Bennett in 2008 Janu- ary-2013 year in January in our hospital in patients with fracture. There were 14 males,3 females,were treated by Orthofix- micro external fixation for the treatment of. According to postoperative healing of the fractures, external fixators were removed time record. After 3 months and 6 months, with upper arm, shoulder, hand function question- naire (disabilities of the arm,shoulder and hand,DASH) score,followed by a total of end-stage joint activity (total action movement,TAM) evaluation. Results 17 patients were followed up,operation time was 20-40 min,average 29.1 min,the amount of intraoperative bleeding 10-35 mL,average 12.5 mL. Postoperative X- films of all patients were healed, including 16 cases obtained anatomical reduction,1 cases of fracture of good to the line, after March, af- ter June, end-stage DASH score two two comparison, the difference was statistically significant (P〈0.05). Follow up of ESRD,TAM score was excellent in 14 cases ,good in 2 cases, poor in 1 cases. The follow-up period without the occurrence of pin tract infection, superficial branch of radial nerve injury and other complications. Conclusion Using Orthofix- mini external fixation for the treatment of fresh fractures of Bennett effect is reliable,the operation is sim- ple, has a certain popularization value.
