危重症儿童低白蛋白血症和低钠血症的临床研究 被引量:3

摘要 目的:了解危重症儿童低白蛋白血症的临床特征和低钠血症的关系。方法:2008年1月—2011年12月我院儿科重症监护病房(pediatric intensive care unit,PICU)收住1 935例患儿,根据其血清白蛋白质量浓度分为低白蛋白组和非低白蛋白组,根据血清钠离子浓度分为低钠血症组和非低钠血症组,将其流行病学、临床理化特征以及预后进行统计分析。结果:低白蛋白组371例患儿中,疾病分布前5位依次是肺炎90例(24.26%),脓毒血症41例(11.05%),脑炎39例(10.51%),外伤39例(10.51%)和肠炎20例(5.39%),低白蛋白血组死亡率(23.72%)显著高于非低白蛋组(5.43%)(P<0.05),并且血清白蛋白水平越低,死亡率越高(P<0.05),低白蛋白组存在低钠血症198例(53.37%),显著高于非低白蛋白组的7.93%(P<0.05),低白蛋白组血清白蛋白质量浓度和血清钠离子浓度呈直线正相关(r=0.66,P<0.05)。结论:儿科重症患儿出现低白蛋白血症与各类重症感染、外伤等因素有关,低白蛋白血症和低钠血症易伴随出现,血清白蛋白水平与临床预后密切相关,要及时发现,并作出针对性的处理。
