针刺联合康复治疗对不同分期脑卒中患者上肢痉挛的临床作用观察

Clinical observation on treating upper limb spasticity in different stages of stroke by acupuncture plus rehabilitation

目的:研究观察针刺联合康复治疗对不同分期脑卒中患者上肢痉挛的作用差别。方法:脑卒中患者63例,急性期患者21例,恢复期患者22例,后遗症期患者20例,均接受常规康复治疗及针刺治疗。治疗前、治疗2周后分别测定痉挛肌肱二头肌肌腹处的表面肌电均方根值(RMS)、Barthel评分。结果:与治疗前比较治疗后3组患者Barthel评分均较前增加(P<0.05),急性期及恢复期患者的RMS值均降低(P<0.05),后遗症期患者RMS值降低不明显(P>0.05)。结论:针刺联合康复治疗对急性期及恢复期脑卒中患者痉挛效果较好,后遗症期效果欠佳。

Objective： To study the difference of acupuncture combined with rehabilitation on upper limb spasticity in different stages of stroke. Methods： 63 cases of stroke were treated by acupuncture combined with conventional rehabilitation. 21 cases of acute phase, 22 cases of recovery phase, 20 cases of sequel stage. The RMS values of biceps surface electromyography and Barthel score were evaluated respectively before the treatment and 2 weeks later. Results： Barthel score of all the three groups increased after treatment （P〈0.05）. The RMS values of both acute and recovery phase reduced （ P〈0.05 ）. The RMS values of sequel stage groups didn＇t reduced （P〉0.05）. Conclusion： The treatment effect of acupuncture combined with rehabilitation therapy is better in the acute and recovery patients of stroke, and is worth in sequel stage patients.
