疏肝温肾痰瘀双解汤配合针灸治疗老年性骨质疏松症腰背痛的临床疗效观察

Curative Observation of Using Shugan Wenshen Tanyu Shuangjie Decociton Combined with Acupuncture-moxibustion to Treat Senile Osteoporosis in Patients with Low Back Pain

目的:观察分析中药疏肝温肾痰瘀双解汤配合针灸治疗老年性骨质疏松症腰背痛的临床疗效。方法:选取我科2011年3月至2014年3月老年性骨质疏松症腰背痛患者100例,随机分为实验组和对照组。对照组予以钙尔奇D、阿法骨化醇和西乐葆治疗,实验组在对照组基础上,加用疏肝温肾痰瘀双解汤配合针灸治疗。对比两组患者治疗前及治疗3个月后的腰椎骨密度、血钙、血清碱性磷酸酶、甲状旁腺素和视觉模拟评分(VAS评分)。对比两组患者治疗3个月后的临床疗效。结果:两组患者治疗3月后的腰椎骨密度和血钙水平均显著高于治疗前,P<0.01;实验组治疗3月后的腰椎骨密度和血钙水平均显著高于对照组,P<0.01。两组患者治疗3个月后的血清碱性磷酸酶、甲状旁腺素和VAS评分均显著低于治疗前,P<0.01;实验组治疗3个月后的血清碱性磷酸酶、甲状旁腺素和VAS评分均显著低于对照组,P<0.01。实验组治疗3个月后的有效率显著高于对照组,χ2=4.8972,P=0.0269<0.05。结论:在常规治疗骨质疏松伴腰背痛的基础上,加用疏肝温肾痰瘀双解汤配合针灸治疗,可显著改善老年患者的临床症状和骨质疏松情况。

Objective： To observe and analyze the clinical efficacy of Shugan Wenshen Tanyu Shuangjie Decoction combined with acupuncture- moxibustion in the treatment of senile osteoporosis with low back pain. Methods： 100 patients of senile osteoporosis and low back pain from March 2011 to March 2014 were randomly divided into experimental and controlled groups. The controlled group received caltrate D,alfacalcidol and celebrex,while the experimental group were given Shugan Wenshen Tanyu Shuangjie Decoction combined with acupuncture- moxibustion on the basis of the controlled group. Lumbar spine BMD serum calcium,serum alkaline phosphatase,parathyroid hormone,and visual analog scale（ VAS score） were compared in two groups of patients before treatment and after 3 months of treatment. The clinical efficacy in two groups of patients was also compared after three months of treatment. Results： The lumbar spine bone mineral density and calcium after three months in two groups of patients were significantly higher than before treatment,P 〈 0. 01; lumbar BMD and serum calcium in experimental treatment of after three months were significantly higher,P 〈 0. 01. The serum alkaline phosphatase parathyroid hormone,and VAS scores three months later in two groups of patients were significantly lower than before treatment,P 〈 0. 01; the serum alkaline phosphatase parathyroid hormone,and VAS scores three months later in experimental group were significantly lower than the controlled group,P 〈 0. 01. The treatment efficiency in the experimental group was significantly higher,χ2= 4. 8972 3 months after,P = 0. 0269 〈 0. 05. Conclusion： In the conventional treatment of osteoporosis associated with low back pain,based on the addition of Shugan Wenshen Tanyu Shuangjie Decoction combined with acupuncture- moxibustion can significantly improve the clinical symptoms and osteoporosis cases of elderly patients.
