宁波镇海区女性健康普查及相应HPV检测分析 被引量:2

摘要 目的通过对宁波镇海区女性宫颈液基薄层细胞学(TCT)健康普查及相应二代基因杂交捕获法高危型人乳头状瘤病毒基因检测(HC2-HPV-DNA),探讨两种技术联合应用在宫颈癌筛查中的价值。方法对3 216例女性行宫颈TCT检查及相应HC2-HPV-DNA检测,并与病理活检结果进行对照分析。结果在3 216例女性中,TCT检查异常者630例,阳性率19.6%(630/3216);在630例TCT检查异常者中HC2-HPV-DNA检测阳性者383例,阳性率60.8%(383/630)。TCT检查及HC2-HPVDNA检测均阳性的383例行宫颈活检,病理诊断为CIN符合率为97.1%;TCT检查为阳性,HC2-HPV-DNA检测为阴性的247例行宫颈活检,病理诊断为CIN符合率为66.4%,经比较差异有统计学意义(x^2=9.40,P<0.01)。结论宫颈TCT检查与宫颈HC2-HPV-DNA检测的联合应用对有效筛查宫颈癌尤为重要。 Objective To evaluate the significance of combined application of TCT and HPV-DNA testing in screening of cervical lesion. Methods TCT and HC2-HPV-DNA testing were performed for 3 216 women and the results were compared to pathological biopsy.Results Totally 630 cases were positive in TCT examination, with positive rate of 19.6%(630/3 216), including 383 cases with positive results in HC2-HPV-DNA testing (60.8%, 383/630).The cases with positive results in TCT examination and HC2-HPV-DNA testing underwent cervical biopsy, and the CIN coincidence rate of pathological diagnosis was 97.1%.There were 247 cases with positive results in TCT examination but negative results in HC2-HPV-DNA testing, and their CIN coincidence rate of pathological diagnosis was 66.4%.The difference in coincidence rate was statistically significant (χ^2 =9.40,P〈0.01).Conclusion It is particularly important to combine TCT examination with HC2-HPV DNA testing in screening of cervical concer.
