摘要
目的比较微柱凝胶法与试管抗人球蛋白试验法检测O型Rh D阳性孕妇血清中Ig G抗-A、抗-B效价的差异,并界定微柱凝胶法检测孕妇Ig G效价的阳性临界值。方法选择2012年8月~2014年9月血型为O型Rh D阳性孕妇血清标本360例,均进行微柱凝胶法与试管抗人球蛋白试验法检测Ig G抗-A、抗-B效价,比较两种检测方法的结果差异,确定微柱凝胶法检测孕妇Ig G效价的阳性临界值。结果微柱凝胶法测定的Ig G抗-A和Ig G抗-B效价几何均数为试管抗人球蛋白试验法测定的2倍;微柱凝胶法检测Ig G抗-A和Ig G抗-B的阳性率为72.8%和67.8%,显著高于试管抗人球蛋白试验法检测的阳性率(45.6%和34.4%)(P〈0.05)。微柱凝胶法与试管抗人球蛋白试验法测定Ig G抗-A和Ig G抗-B效价结果均呈高度正相关(rA=0.869,rB=0.912,P〈0.05),根据回归方程计算微柱凝胶法具有临床意义的Ig G抗-A和Ig G抗-B效价的阳性临界值分别为128和121。结论微柱凝胶法检测孕妇Ig G效价较试管抗人球蛋白试验法更简便、快捷,灵敏度高。对其阳性临界值的界定,可为临床诊断提供可靠的依据。
Objective To make comparison between the differences in the detection of IgG anti-A and anti-B titer in the serum of pregnant women with O-blood type and positive RhD by the methods of micro column gel and vitro anti-human globulin test respectively, and to define the positively critical values of the micro column gel method for the detection of pregnant women's IgG titer. Methods Between August 2012 and September 2014, there were 360 pregnant women with the O blood type and positive RhD whose serum samples were chosen to detect the IgG anti-A and anti-B titer by the methods of micro column gel and vitro anti-human globulin test respectively. Comparison was made between the results of the two methods. The positively critical value of the micro column gel method for the detection of pregnant women's IgG titer was defined. Results The geometric mean of IgG anti-A and anti-B titer detected by micro column gel method was twice than that detected by vitro anti-human globulin test. The positive rates of IgG anti-A and anti-B titer detected by micro column gel was 72.8% and 67.8%, which were significantly higher than those detected by vitro anti-human globulin test (45.6% and 34.4%) (P 〈 0.05). The results of IgG anti-A and anti-B titer detected by both method all had a high positive correlation (rA =0.869, rB =0.912, P 〈 0.05). The positively critical values of the IgG anti-A and anti-B titer were 128 and 121 according to the regression equation calculation method of micro column gel. Conclusion As to the detection of pregnant women's IgG blood group, micro column gel method is more convenient and quicker and has higher sensitivity than vitro anti-human globulin test. The definition of the positively critical value by the former method can provide a reliable basis for the clinical diagnosis.
