摘要
目的 分析双源CT扫描在淋巴瘤与淋巴转移瘤中鉴别诊断中的应用。方法 回顾性分析2012年1月-2013年12月35例淋巴瘤患者和33例恶性肿瘤淋巴结转移患者的临床资料,所有患者均采集双能量CT图像,进行处理后得到两组患者动静脉期的碘图、能量衰减曲线等,测量两组淋巴结在碘图的碘浓度、碘覆盖值、病变同层大血管碘浓度等,测量两组患者平扫、混合及单能谱图像上病变淋巴结的CT结果。分析双源CT双能量扫描在淋巴瘤与淋巴转移瘤鉴别诊断中的应用价值。结果 淋巴瘤组患者CTP、CTAE、CTVECT值水平均显著高于转移瘤组,差异有统计学意义(P〈0.01)。淋巴瘤组碘分布图相关能量参数CTA、CTV、ICAP、ICVP、NICVP显著高于转移瘤,NICAP显著低于转移瘤,差异有统计学意义(P〈0.01)。不同单能量keV下淋巴瘤和转移瘤淋巴结CT值比较,淋巴瘤CT值显著高于转移瘤组,差异有统计学意义(P〈0.01)。淋巴瘤和转移性淋巴结不同单能量keV下病变淋巴结CT值变化趋势不同。结论 双源CT扫描与普通扫描不同,能够提供淋巴结形态学变化以及功能代谢状态,为淋巴瘤及淋巴转移瘤的鉴别诊断提供新的思路。
Objective To discuss application of Dual-source CT scan in differential diagnosis of lymphoma and lymphatic metastases. Methods Clinical data of 35 cases with lymphoma and 33 cases with lymphatic metastases from Jan 2012 to Dem 2013 were respectively analyzed. Dual-source CT scan image of two groups were analyzed in order to get arterial phase and venous phase diagram of iodine, energy attenuation curve, and so on. Iodine concentration, iodine coverage values and macrovascular disease with iodine concentration layer of lymph nodes of two groups were measured. Value of dual-source CT scan in differential diagnosis of lymphoma and lymphatic metastases was analyzed.Results The CTP, CTAE, CTVElevels of lymphoma group were higher than lymphatic metastases group(P〈0.01). CTA, CTV, I CAP, ICVP, NICVPin iodine distribution image of lymphoma group were higher than lymphatic metastases group, and NICAPwas lower than lymphatic metastases group(P〈0.01). CT values in different single energy ke V of lymphoma group were higher than lymphatic metastases group( P〈0.01). Lymph node lesions CT value change different trend of two groups in different single energy ke V was different. Conclusion Dual-source CT scan is different with ordinary CT scanning, which provides morphological changes and functional metabolism of lymph node status, and provides a new way of thinking for differential diagnosis of lymphoma lymphatic metastases.
