计算机认知矫正治疗对精神分裂症患者认知功能的影响 被引量:20

Effects of computer cognitive remediation therapy on cognitive function in patients with schizophrenia

摘要 目的:探讨计算机认知矫正治疗(CCRT)对精神分裂症患者认知功能的影响。方法84例精神分裂症患者随机分为CCRT组和对照组,每组各42例。两组在药物治疗的基础上,CCRT组给予计算机认知矫正辅助治疗,对照组只给予药物治疗。分别在治疗前、治疗12周后进行阳性与阴性症状量表(PANSS)、威斯康星卡片分类测验(WCST)、精神分裂症简明认知功能成套测评量表(BACS)的评估。结果12周治疗后,CCRT组患者的代币运动(TM)[(69.29±14.88)比(62.78±8.15),t=2.40,P<0.05]、言语记忆(VM)[(51.24±8.24)比(38.31±11.63),t=5.67,P<0.01]、伦敦塔(TOL)[(14.94±4.45)比(8.68±5.12),t=5.76,P<0.01]、总应答数(Ra)[(88.85±15.32)比(107.34±18.11),t=4.87,P<0.01]、错误应答数(Re)[(52.56±20.32)比(65.61±16.48),t=3.12,P<0.01]、持续性错误(Rpe)[(24.77±6.80)比(37.22±13.22),t=5.23,P<0.05]和治疗前比较,成绩改善,差异有统计学意义;并且伦敦塔(TOL)[(14.94±4.45)比(11.23±2.52),t=4.44,P<0.01]、持续性错误[(Rpe)(24.77±6.80)比(31.31±6.20),t=4.37,P<0.01]成绩优于对照组,差异有统计学意义。结论计算机认知矫正辅助治疗能有效改善精神分裂症患者的认知功能。 Objective To evaluate the efficiency of computer cognitive remediation therapy (CCRT) on improving the cognitive function in patients with schizophrenia .Methods A total of 84 pa‐tients with schizophrenia in stable condition were randomly divided into CCRT group (n = 42) and the control group (n= 42) .On the basis of drug therapy ,the CCRT group received computer-assisted cog‐nitive remediation therapy ,the control group received drug treatment only .Before treatment and after treatment for 12 weeks ,we respectively carried on the Positive and Negative Syndrome Scale(PANSS) , Wisconsin Card Sorting Test(WCST) ,and Brief Cognitive Assessment Scale in Schizophrenia (BACS) to assess the patients .Results Compared with before treatment ,after treatment for 12 weeks ,there was a significant improvement in the score of CCRT group ,including TM [(69 .29 ± 14 .88)vs(62 .78 ± 8 .15) , t = 2 .40 ,P 〈 0 .05 ] ,V M [(51 .24 ± 8 .24 )v s (38 .31 ± 11 .63 ) , t = 5 .67 ,P 〈 0 .01 ] ,T O L [(14 .94 ± 4.45)vs(8 .68 ± 5 .12) ,t = 5 .76 ,P 〈 0 .01] ,Ra[(88 .85 ± 15 .32)vs(107 .34 ± 18 .11) ,t = 4 .87 ,P 〈0.01] ,Re[(52 .56 ± 20 .32)vs(65 .61 ± 16 .48) ,t = 3 .12 ,P 〈 0 .01] and Rpe [(24 .77 ± 6 .80)vs(37 .22 ± 13 .22) ,t= 5 .23 ,P〈 0 .05] ,the difference was statistically significant .The scores of TOL [(14 .94 ± 4 .45)vs(11 .23 ± 2 .52) ,t= 4 .44 ,P〈 0 .01] and Rpe [(24 .77 ± 6 .80)vs(31 .31 ± 6 .20) ,t= 4 .37 ,P〈0 .01] were remarkablely higher than that of the control group ,the difference was statistically signifi‐cant .Conclusions CCRT can improve the cognitive function of patients with schizophrenia efficiently .
