小儿反复呼吸道感染血清微量元素及体液免疫的变化与分析 被引量:25

摘要 目的探讨反复呼吸道感染小儿血微量元素以及免疫指标的变化。方法选取反复呼吸道感染(RRI)患儿104例(观察组),同时选取健康体检儿童98例作为对照(对照组),检测两组血清微量元素以及免疫指标。结果观察组血清Ca、Zn和Fe含量分别为(0.79±0.31)mmol/L、(60.14±9.58)mmol/L和(5.20±1.03)mmol/L,明显低于对照组的(1.64±0.43)mmol/L、(113.02±8.74)mmol/L和(9.79±0.74)mmol/L,差异有统计学意义(P<0.05);观察组和对照组血清Mg和Cu含量差异比较无统计学意义(P>0.05);观察组lg G和lg A水平分别为(3.89±0.67)g/L和(0.32±0.20)g/L,明显低于对照组的(9.71±0.93)g/L和(0.81±0.19)g/L,差异有统计学意义(P<0.05);观察组和对照组lgM、CD_3^+、CD_4^+、C3和C4差异比较无统计学意义(P>0.05);血清Mg、Ca、Fe、Cu与免疫指标水平无相关性,而血清Zn与lg G、lg A呈正相关(r=0.842和0.899,P<0.05)。结论反复呼吸道感染小儿血清Zn、Ca、Fe以及lg G、lg A免疫球蛋白水平降低,其中低Zn可能是造成免疫指标低下的原因。 Objective To investigate the changes of serum trace element level and humoral immunity in children with recurrent respiratory tract infection. Methods 104 children with recurrent respiratory tract infection from January 2013 to December 2014 were selected as the observation group and 98 healthy children as the control group. The serum trace elements and immune parameters were detected. Results The concentration of serum Ca, Zn and Fe was (0. 79 ± 0. 31) mmol/L, (60. 14 ± 9. 58) mmol/L and (5. 20 ± 1. 03) mmol/L in the observation group, which were significantly lower than those in the control group [(1. 64 ± 0. 43) mmol/L, (113. 02 ± 8. 74) mmol/L and (9. 79 ± 0. 74) mmol/L] (P〈0. 05). There was no significant difference in serum Mg and Cu between the two groups (P〉0. 05). The levels of lgG and lgA were (3. 89 ± 0. 67) g/L and (0. 32 ± 0. 20) g/L in the ob-servation group, and (9. 71 ± 0. 93) g/L and (0. 81 ± 0. 19) g/L in the control group (P〈0. 05). The levels of lgM, CD3+, CD4+, C3 and C4 had no statistical significance between the two groups (P〉0. 05). Immune index had no correlation with the levels of serum Mg, Ca, Fe and Cu, but it was positively correlated with serum Zn, lgA and lgG (r=0. 842 and 0. 899, P〈0. 05). Conclusion The levels of serum Zn,Ca and Fe and the immunoglobulin levels of IgG and IgA decrease in children with recurrent respiratory tract infection, and the low level of Zn is the pos-sible cause of low immune index.
