针灸治疗早期2型糖尿病神经病变的临床观察

Clinical Observation on the Effect of Acupuncture Treatment for Early Type 2 Diabetic Neuropathy Patients

目的观察针灸对早期2型糖尿病周围神经病变患者的临床疗效。方法在血糖控制理想的情况下,采用自身前后对照及针灸治疗组与药物治疗组之间比较的临床试验方法,将60例患者根据随机数字表按就诊顺序随机分为2组,其中针灸治疗组30例,药物治疗组30例,男女比例随机分配。选用透刺法,药物治疗组以改善循环、营养神经等药物治疗。疗程结束后记录各项观察指标,并统计疗效。治疗结束后分别检测正中神经、腓总神经传导速度,记录治疗前后糖尿病周围神经病变积分,并评价疗效。结果针灸治疗组与药物治疗组总有效率分别为90.0%和63.3%,差异有统计学意义(P<0.05);治疗后针灸治疗组正中神经、腓总神经传导速度(MCV)及感觉神经传导速度(SCV)均高于药物组(P<0.05,P<0.01);糖尿病周围神经病变积分亦低于药物组分别为:(3.9±0.8)(5.4±1.1,P<0.05)。结论针灸治疗早期2型糖尿病周围神经病变有很好的疗效,值得临床推广应用。

Objective： To observe the clinical effect of acupuncture treatment for peripheral neuropathy patients with early type 2 dia- betes. Methods ： In the case of ideal glycemic control, clinical test methods were used to compare self - control acupuncture group and drug group before and after treatment. 60 patients were randomly divided into an acupuncture group and a drug group according to their visiting sequence numbers, 30 cases per group, with a random proportion of male and female patients. The？ acupuncture group was treated with penetration needling method while the drug group was given drugs for circulation improvement and nutrition nerve. After the treatment, each indicator was recorded and observed, and the clinical effectiveness was counted. And the median nerve and peroneal nerve conduction velocity were detected after the treatment. The diabetic peripheral neuropathy scores were recorded before and after the treatment and the curative effect was evaluated. Results： The total effectiveness of the acupuncture treatment group and the drug treatment group was 90. 0% and 63. 3% respectively, and the difference was statistically significant （P 〈0. 05）. The median nerve and peroneal nerve conduction ve- locity of the acupuncture group were higher than that of the drug group （ P 〈 0. 05, P 〈 0. 01 ）, and the diabetic peripheral neuropathy scores of the acupuncture group were also lower than that of the drug group, （3.9 ± 0. 8） （5.4± 1.1, P 〈 0.05）. Conclusion： Acupunc- ture treatment has a better effect for early type 2 diabetic peripheral neuropathy patients, worthy of clinical application.
