基于中医养生理论的辨证施护对缺血性脑卒中恢复期患者的影响

Effects of dialectical nursing for ischemic stroke patients in restoration stage by traditional Chinese medicine regime theory

目的探讨根据中医养生理论制订的中医辨证施护措施对缺血性脑卒中恢复期患者康复效果的影响。方法选取2010年12月至2014年8月在西安某医院住院的缺血性脑卒中恢复期患者108例,实验组与对照组各54例。对照组采用常规护理方法,实验组在常规护理的基础上采用根据中医养生理论制订的辨证施护措施,并比较两组患者神经功能、生活自理能力及生活质量。结果实验组与对照组入院时美国国立卫生研究院卒中量表(National Institute of Health Stroke Scale,NIHSS)评分、生活质量评价量表-12(12-item Medical Outcomes Study Short Form,SF-12)评分、改良RANKIN量表(Modified Rankin Scale,MRS)评分及巴氏指数(the Barthel Index of ADL,BI)无明显差异。进行护理干预后,观察组NIHSS评分明显降低,SF-12评分、MRS评分及生活自理能力明显增高(P<0.05),与对照组相比,差异具有统计学意义(P<0.05)。结论根据中医养生理论进行辨证施护可以有效改善恢复期缺血性脑卒中患者肢体运动功能及生活质量,提高康复效果。

Objective To explore the effects of dialectical nursing for patients with ischemic stroke in restoration stage by traditional Chinese medieine（TCM） regime theory. Methods A total of 108 ischemic stroke patients in restoration stage were randomly divided into the experimental group（n=54） and the control group（n=54）. The control group was given routine nursing care,and the experimental group was given the dialectical nursing intervention based on the TCM regime theory. The nerve function and activities of daily living among patients were evaluated by NIHSS score,quality of life scale （SF-12）,MRS score,and modified Barthel index respectively before and after the nursing measures. Results There was no significant difference in NIHSS score,SF-12 score,MRS score and modified Barthel index between the experimental group and control group before nursing measures. The NIHSS score of the experimental group after nursing was significantly lower than that in the control group and the differ- ence was statistically significant（P〈0.05）. The SF-12 score,MRS score and modified Barthel index of the experimental group after intervention were significantly higher than those of the control group and the difference was statistically significant （P〈0.05）. Conclusion The dialectical nursing based on TCM regime theory could improve the recovery of extremity motor function,quality of life for patients with ischemic stroke in restoration stage and promote rehabilitation.
