药物流产后口服优思明的临床价值研究

Clinical value research of using yasmin after medical abortion

目的探讨药物流产后口服优思明的临床价值。方法选取2014年7月至2015年4月在潍坊医学院附属医院产科门诊行药物流产的120例未育年轻女性患者为研究对象,按就诊先后顺序分为观察组和对照组各60例。两组均采用米非司酮联合米索前列醇进行药物流产,对照组在孕囊排出后常规口服头孢氨苄预防感染和产后逐瘀胶囊促子宫复旧,观察组在对照组基础上于孕囊排出后开始口服优思明,每天1片(每片含屈螺酮3mg和炔雌醇0.03mg),连服21d,来月经后继服下一周期,共服3个周期。比较两组阴道出血量及出血持续时间、药流后清宫率及月经恢复情况。结果药物流产后,观察组阴道出血量及出血持续时间、药流后清宫率均低于对照组,月经周期恢复情况观察组优于对照组,两组比较差异有统计学意义(P<0.05)。结论药流后口服优思明,不仅可以减少阴道出血量及出血时间,降低药流后清宫率,而且能尽快恢复月经,维持正常月经周期,对药物流产患者减少术后并发症具有重要意义。

Objective To investigate the clinical value of using yasmin after medical abortion.Methods A total of 120 patients who accepted medical abortion in obstetric clinic of Affiliated Hospital Weifang Medical College from July 2014 to April 2015 were selected as the research objects.According to the doctor order,they were divided into the observed group and the control group,with 60 cases in each group.Both groups used mifepristone combined misoprostol for medical abortion.After pregnancy sac discharge cephalosporins ammoniac benzyl and postptpartum stasis capsuleuse were used regularly in both groups to prevent infection and to promote uterine instauration.At the same time,yasmin（including drospirenone 3mg and ethinylestradiol0.03mg）were used after pregnancy sac discharge in the observed group in the first stage（1tablet qd po,for 21days）,taking the next cycle after menstruation for 3cycles.Vaginal bleeding,bleeding time,curettage situation and menst-ruation recovery time were compared in both groups.Results After medical abortion,vaginal bleeding,bleeding time and curettage situation of observed group were lower than that of control group,and the menstruation recovery time in the observed group was better than that in the control group.Differences between the two groups had statistical significance（P〈0.05）.Conclusion Using yasmin after medical abortion can effectively reduce the amount of vaginal bleeding,shorten bleeding time and curettage situation,promote the recovery of the menstrual cycle and maintain normal menstrual cycle,which is of great significance in reducing the postoperative complications in patients with medical abortion.
