退银汤对中重度寻常性银屑病患者外周血及皮损中CCR4表达的影响

Effect of Tuiyin Soup on CCR4 in peripheral blood and skin lesions of psoriasis

目的:探讨趋化因子受体4(CCR4)在中重度寻常性银屑病患者与健康人外周血及皮损组织液的表达差异,及其与银屑病皮损面积和严重程度指数(PASI)的相关性,探讨退银汤在寻常性银屑病治疗中的作用机制。方法:采用流式细胞仪检测40例中重度寻常性银屑病患者在使用退银汤治疗前后外周血CCR4在T细胞表达率,与40例正常对照组进行比较;另采用酶联免疫吸附法检测40例寻常性银屑病患者在使用退银汤治疗前后皮损组织液中CCR4表达水平,与40例正常对照组进行比较。结果 :中重度寻常性银屑病患者外周血CCR4在T细胞表达率治疗前与正常对照组相比较,差异有统计学意义(P<0.05);经退银汤治疗12周后,CCR4在T细胞表达率未见明显变化,差异无统计学意义(P>0.05)。皮损组织液中CCR4表达水平在治疗前高于正常对照组,经退银汤治疗12周后,CCR4表达水平明显低于治疗前,差异均有统计学意义(P<0.05),并患者相应的PASI呈明显正相关性。结论:退银汤可以降低中重度寻常性银屑病患者皮损组织液中CCR4表达水平,且CCR4随病情而变化,而外周血T细胞CCR4表达率则与病情无明显相关性。

Objective：To study the impact of expression level of CCR4 on T lymphocytes in peripheral blood and lesions of psoriasis vulgaris who received the Tuiyin Soup therapy. Methods：By using the flow cytometric analysis to detect the expression rate of peripheral T lympocytes of CCR4 of 40 cases of psoriasis vulgaris before and after treatment in patients with Tuiyin Soup,and 40 healthy people were also detected. Double antibody sandwich-enzyme linked with immunosorbent assay was used to detect peripheral blood levels of CCR4 in 40 cases of psoriasis vulgaris before and after treatment with Tuiyin Soup,and 40 cases of the control group were also detected. Results： The expression rate of peripheral T lympocytes of CCR4 of 40 cases of psoriasis vulgaris before treatment was obviously higher than that of the healthy people（P〈0.05）, while there showed no difference between the patients before and after treatment by 12 weeks of Tuiyin soup（P〉0.05）. The expression level of CCR4 in the Skin lesions of the patients with psoriasis vulgaris was obviously higher than that of the control group, after 12 weeks of treatment with Tuiyin soup, the expression level of CCR4 in the Skin lesions of the patients decreased, and the differences were both significant（P〈0.05）.And the level of CCR4 of the Skin lesions was correlated with the areas and severity index（PASI） of the patients. Conclusion：Tuiyin Soup can reduce the level of CCR4 in the Skin lesions of patients with psoriasis vulgaris, while there was no correlation between the expression level of CCR4 on T lymphocytes in peripheral blood.
