CT、MRI诊断卵巢癌价值的临床研究 被引量:22

Evaluation on clinical application of CT and MRI in the diagnosis of ovarian cancer

摘要 目的探讨CT与MRI诊断卵巢癌的临床应用价值。方法以我院收治的62例卵巢癌患者作为研究对象,对其CT、MRI资料进行回顾性分析。以病理检查结果为金标准,对比分析两种诊断方式的诊断符合率。结果 CT诊断52例,诊断符合率为83.9%;MRI诊断59例,诊断符合率为95.2%,MRI诊断符合率明显高于CT(χ2=4.17,P=0.041);CT定性诊断符合率为72.6%,低于MRI的88.7%,差异有统计学意义(χ2=5.12,P=0.023 6);MRI分期诊断符合率为90.3%,侵袭和转移的诊断符合率为87.1%,均高于CT(74.2%、72.6%),差异有统计学意义(χ2=5.48,P=0.019 2;χ2=4.02,P=0.044 8)。结论 MRI诊断卵巢癌的临床价值较高,与CT相比,MRI对卵巢癌的定性、分期及肿瘤侵犯周围器官和转移的诊断更佳;临床诊断时,可将两者联合使用,以提高卵巢癌的诊断准确性。 Objective To investigate the clinical value of CT and MRI in diagnosis of ovarian cancer. Methods Sixty two patients with ovarian cancer admitted in the hospital were selected as research subjects, who were retrospectively analyzed according to imaging data(CT and MR images). Pathological examination results were treated as gold standards, and diagnostic accuracy rates of two diagnosis ways were compared and analyzed. Results Fifty two patients were confirmed via CT with diagnostic accuracy rate at 83.9%; 59 patients were confirmed via MRI with diagnostic accuracy at 95.2%, and the diagnostic accuracy of MRI was significantly higher than that of CT(χ2=4.17, P=0.041). The qualitative diagnostic accuracy of CT was 72.6%, being lower than that of MRI at 88.7%, and the difference was significant(χ2=5.12, P=0.023 6). The diagnosis rate of MRI staging was 87.1%, being higher than that of CT at 72.6%, and the difference was significant(χ2=5.48, P=0.019 2; χ2=4.02, P=0.044 8). Conclusions MRI delivers high clinical value in the diagnosis of ovarian cancer, and the diagnosis of MRI on qualitative determination and staging of ovarian cancer and the invasion of tumor over surrounding organs is better. CT and MRI may be combined in clinical diagnosis practices in order to improve the diagnostic accuracy of ovarian cancer.
