摘要
目的:探讨哮喘患者外周血单个核细胞(PBMCs)中白细胞介素23(IL-23)、白细胞介素17(IL-17)、肿瘤坏死因子α(TNFα)、转化生长因子β(TGFβ)、Foxp3(Foxp3)基因mRNA的表达及其临床意义。方法:以实时定量逆转录聚合酶链式反应(RT-PCR)分别检测68例急性发作期哮喘患者及60例健康体检者PBMCs中IL-23、IL-17、TNFα、TGFβ、Foxp3基因mRNA的表达情况,并进一步分析其内在关联性。结果:与对照组相比,哮喘急性发作期患者PBMCs中的IL-23和TNFα mRNA表达水平显著增高(P<0.05),TGFβ mRNA表达水平显著下降(P<0.05);而IL-17、Foxp3 mRNA表达水平差异无统计学意义(P>0.05);TNFα表达水平分别与IL-17、IL-23呈正相关性,而Foxp3与TGFβ呈正相关性;IL-23 mRNA水平与哮喘患者病程、疾病严重程度和疾病控制程度存在一定关联性。结论:IL-23/Th-17轴在哮喘炎症反应过程中可能发挥潜在的重要作用,IL-23与哮喘的严重程度密切相关,有望作为其治疗监测、预后转归的潜在生物评价指标。
Objective:To investigate the levels of the mRNA expression of IL-23,IL-17,TNFα,TGFβand Foxp3 gene in peripheral blood monocytes(PBMCs)of asthma patients and their clinical significance.Method:The mRNA expression of IL-23,IL-17,TNFα,TGFβ and Foxp3 in PBMCs of 68 asthma patients and 60 healthy controls were detected using fluorescence quantitative real-time reverse transcription-polymerase chain reaction,and the correlation between them were analyzed.Result:Compared to the normal group,the mRNA expression of IL-23 and TNFαin PBMCs of asthma patients was significantly increased(P〈0.05),and TGFβ was significantly decreased(P〈0.05),while the expression of IL17 and Foxp3 had no significance between the patients group and the normal group(P〉0.05).The mRNA expression of TNFαwas positively correlated to IL-17 and IL-23,so as TGFβ to Foxp3.In addition,the expression level of IL-23 mRNA was correlated to the duration of disease,disease severity and disease control in asthma patients.Conclusion:IL-23/Th-17 axis might play an important role in asthma inflammation,and IL-23 may be closely related to the severity of asthma,which is expected as one biomark of its evaluation and treatment.
