摘要
目的研究LED红蓝光联合照射对创伤后引起的慢性伤口愈合的临床效果。方法采用随机分层方法以伤口类型为分层将伤口分为试验组(蓝光照射+红光照射,n=18)和对照组(红光照射,n=18)。2组伤口其他处置方案一致,规定10 d为1个疗程。治疗第1、3、5、7、10天分别测量每组伤口的疼痛评分、照射前后温度变化、伤口面积并计算创面缩小率与创面相对愈合速率,并观察有无创面局部和全身的不良反应。结果光照射治疗1个疗程后,对照组和试验组的伤口面积以及伤口疼痛评分均明显减小(时间主效应P<0.05),所有病例未出现伤口局部及全身的不良反应。与对照组相比,红蓝光联合照射可显著提高创面面积缩小率(P<0.001)、创面相对愈合速率(P=0.006)、创面愈合的疗效(P=0.010),且更有效地减轻创面疼痛(P=0.008)。结论红蓝光联合使用比单纯红光更能有效促进伤口愈合、减轻伤口疼痛。连续干预10 d是安全、有效的。
Objective To investigate the clinical effect of light-emitting diode( LED) red light and blue light combined irradiation on healing chronic traumatic wounds. Methods With a random stratification method based on the wound types,the 36 patients with chronic traumatic wounds admitted in Daping Hospital from June 2014 to November 2015 were divided into the control group( n = 18) receiving red light therapy,and the experimental group( n = 18) receiving red light and blue light combined therapy. Wounds in both groups were treated with unified disposal options except LED irradiation,and the therapy lasted for 10 d as a course. The pain scores,wound areas and temperatures before and after irradiation were measured on the 1st,3rd,5th,7th and 10 th days. The wound contraction ratio and wound relative healing rate were calculated.The local or systemic adverse reactions were observed. Results After irradiation for 1 course,the wound areas and pain scores of both groups significantly decreased and presented a significant time effect( P〈 0. 05). Moreover,no local and systemic adverse reactions were observed in all cases. Compared with the control group,the red light and blue light combined irradiation significantly improved the wound contraction ratio( P〈 0. 001),wound relative heating rate( P = 0. 006),and wound healing efficacy( P = 0. 01),and effectively relieved the pain of wounds( P = 0. 008). Conclusion Compared with the red light irradiation,the red light and blue light combined irradiation is more effective in assisting the healing and relieving the pain of chronic traumatic wounds,and 10-day irradiation therapy is safe.
