厚朴三物汤化裁对脾胃壅滞证2型糖尿病患者降糖效果及炎性因子水平的影响

Influence of Hypoglycemic Effect and Inflammatory State for Treating Spleen and Stomach Stagnation Syndrome Diabetes Patients with Houpo Sanwu Decoction

目的:探究厚朴三物汤化裁对脾胃壅滞证2型糖尿病患者的降糖效果,重点观察其对患者炎性因子水平的影响。方法:将我院收治的脾胃壅滞证2型糖尿病患者120例随机分为对照组和观察组患者各60例,对照组患者给予盐酸二甲双胍片内服,观察组患者则加用厚朴三物汤化裁内服,15 d为1个疗程,连续治疗4个疗程。观察两组患者的临床症状,评价证候积分,监测血糖指标、胰岛素抵抗的变化,计算有效率,检测患者炎性因子水平的变化情况。结果:观察组患者临床症状改善优于对照组患者,其证候积分显著低于对照组患者,有效率91.7%较对照组有效率76.7%显著增高(P<0.05);观察组患者的降糖效果明显优于对照组患者,胰岛素抵抗亦较对照组显著改善,其血糖FPG、2h PG、Hb A1C、FINS、HOMA-IR水平较对照组患者显著降低(P<0.05);观察组患者炎性因子hsCRP、IL-6、TNF-α水平较对照组患者显著降低(P<0.05)。结论:厚朴三物汤化裁具有良好的降糖效果,能缓解患者症状,其机制可能与降低炎性因子水平有一定相关性,值得临床深入研究推广。

Objective：To investigate the clinical efficacy of treating spleen and stomach stagnation syndrome diabetes patients with Houpo Sanwu Decoction, and explore the influence of hypoglycemic effect and inflammatory state. Methods： 120 2 diabetes patients were randomly divided into a control group of 60 cases and an observation group of 60 cases. Patients in the control group were treated with metformin hydroehloride tablets ; while patients in the observation group were treated with Houpo Sanwu Decoction, 15 days for a course, 4 courses for treatment. The research was aimed at observing the improvement of the main clinical symptoms, evaluating its syndrome integral, monitoring the changes of glucose and insulin levels, observing the efficacy of the drug, and testing the inflammatory factor levels. Results ： The improvement of main elinieal symptoms for the patients in the observation group was better than that for the patients in the eontrol group, the symptoms score was shorter than that of the control group, and the efficiency was 91.7%, which was higher than that of the control group 76.7% （P〈0.05）; observation group of patients with hypoglycemie effect was better than that of the control group patients, and the insulin resistance was also improved more signifieantly than that of the control group patients, the levels of FPG, 2 hPG, HbA1C, FINS and HOMA-IR were lower than that of the control group patients （ P〈0.05 ）; the levels of hs-CRP, IL-6 and TNF-α were lower than that of the control group patients （ P〈0.05 ）. Conclusion ： Houpo Sanwu Decoction has good hypoglycemic effect, it can improve the patients＇ clinical symptoms, the mechanism may be assoeiated with reducing the levels of inflammatory factors, is worth of wide applieation in the clinical practice.
