一次性根管治疗术对急性牙髓炎的临床效果

Clinical effect of one-off root canal therapy for acute pulpitis

目的观察一次性根管治疗术与常规根管治疗术治疗急性牙髓炎的近远期效果。方法选择2013年1月～2015年1月南阳市第九人民医院口腔科收治的急性牙髓炎患者200例作为研究对象，患牙200颗．按随机原则分为一次性组与常规组，每组各100例，患牙各100颗。一次性组给予一次性根管治疗术，常规组给予常规根管治疗术。比较两组疼痛情况，治疗后1年患者满意度，及治疗后2周和1年两组的近、远期治疗效果。结果一次性组与常规组术后疼痛水平差异无统计学意义（P〉0．05）；治疗后1年，一次性组的满意度得分为（89．12±15．21）分，常规组为（71．68±13．57）分，一次性组满意度显著高于常规组（P〈o．05）；治疗后2周，一次性组患者近期显效率为87．0％，常规组为55．0％，两组比较差异有统计学意义（P〈0．05）；两组患者近期总有效率、远期成功率和总有效率比较差异均无统计学意义（P〉0．05）。结论根管治疗术是治疗急性牙髓炎的有效方法，一次性根管治疗和常规根管治疗均具有显著的远期疗效，但一次性根管治疗少了患者的复诊次数，提高了患者依从性，值得临床推广。

Objective To observe the short-term and long-term effects of one-off root canal therapy and conventional root canal therapy for acute pulpitis. Methods 200 cases of patients （200 teeth） with acute pulpitis treated in Depart- ment of Stomatology, the Ninth People＇s Hospital of Nanyang City from January 2013 to January 2015 were selected as research objects. According to the principle of random, patients were divided into the one-off group and the conven- tional group, each group had 100 cases （100 teeth）. Patients in the one-off group were given one-off root canal therapy, and patients in the conventional group were given conventional root canal therapy. The pain level, satisfaction of 1 year after treatment, short-term and long-term effects of the two groups were observed. Results The pain level between the two groups had no significant difference （P 〉 0.05）. After 1 year of treatment, the satisfaction score of the one-off group was （89.12±15.21） points, which of the conventional group was （71.68±13.57） points, with statistically significant difference （P 〈 0.05）. After 2 weeks of treatment, the recent effective rate of the one-off group was 87.0%, which of the conventional group was 55.0%, with statistically significant difference （P 〈 0.05）. There were no statistically significant dif- ferences in recent total effective rate, long-dated success rate and total effective rate between the two groups （P 〉 0.05）. Conclusion Root canal is an effective way for acute pulpitis treatment. One-off root canal therapy and conven- tional root canal therapy all have significant long-term effects, but one-off root canal therapy reduce the appointment times of patients, enhance the patients＂ compliance, which is worthy of clinical application.
