TLR4Asp299Gly基因多态性与炎症性肠病易感性的Meta分析

Association between TLR4 Asp299Gly polymorphism and susceptibility of inflammatory bowel disease： a Meta-analysis

目的评价TLR4Asp299Gly基因多态性与炎症性肠病（IBD）的易感性。方法检索PubMed、CNKI、万方、CBM和维普等数据库，查找Toll样受体4（TLR4）Asp299Gly基因与IBD相关的病例对照研究。应用RevMan5．2和Stata12．0软件进行Meta分析。结果最初检索到318篇文献，根据纳入标准和排除标准后纳入19篇文献，其中克罗恩病（CD）相关研究17篇，溃疡性结肠炎（UC）相关研究12篇，共纳入5785例患者和3729例对照者。Meta分析结果显示：与基因型AA比较，基因型AG、GG、AG＋GG均增加CD的风险；与等位基因A比较，等位基因G增加CD的风险。与基因型AA比较，基因型AG、GG、AG＋GG均增加溃疡性结肠炎（UC）的风险；与基因型AG＋AA比较，基因型GG增加UC的风险；与等位基因A比较，等位基因G增加UC的风险。TLR4多态性与欧洲人群IBD及亚洲人群UC相关。结论TLR4Asp299Gly基因与IBD易感性相关，等位基因G可增加IBD的风险。

Objective To systematically review TLR4 Asp299Gly polymorphism and the susceptibility of inflammatory bowel disease （IBD）. Methods All eligible case-control studies were identified by searching PubMed, CNKI, WanFang Data, CBM, VIP and so on. Then, Meta-analysis was performed by using RevMan 5.2 and Stata 12.0 software. Results A total 318 papers had been searched originally,but only 19 papers were included according to inclusion criteria and exclusion criteria. There were 17 papers about crohn disease （CD） and 12 papers about ulcerative colitis （UC） , and 5 785 patients and 3 729 controls were included. Compared with the wild genotype AA, genotype AG, GG and AG ＋ GG increased CD risk;compared with allele A, allele G increased CD risk. Compared with genotype AA, genotype AG, GG and AG ＋ GG increased UC risk; compared with genotype AG ＋ AA, genotype GG increased UC risk;compared with allele A, allele G increased UC risk. TLR4 polymorphism was associated with IBD in Europeans while polymorphism was associated with UC in Asians. Conclusion TLR4 Asp299Gly gene is associated with susceptibility of IBD. Allele G may increase the risk of 1BD.
