经皮冠状动脉介入治疗对多支血管病变患者心脏功能的影响

Clinical Effect of PCI in the Cardiac Function for Patients with Multi-vessel Disease

目的研究探讨经皮冠状动脉介入治疗(PCI)对冠心病多支血管病变患者心脏功能的影响。方法方便选取并回顾性分析60例2013年1月—2015年1月在该院行冠状动脉造影(CAG)明确诊断冠心病伴多支血管病变患者,根据患者是否接受PCI治疗分为观察组与对照组,每组30例。对照组患者单纯予常规药物治疗(抗血小板聚集、调脂稳定斑块、硝酸酯类、ACEI/ARB、β受体阻滞剂等),观察组患者在予以常规治疗的基础上联合PCI行完全或部分血运重建,观察与比较两组患者血清BNP水平、超声心动图指标以及1年内因再发心肌缺血事件或心衰再住院率。结果与对照组相比,观察组患者血清BNP水平(196.17±16.26)ng/L显著低于对照组患者(223.08±15.34)(P<0.05);超声心动图指标的改善显著优于对照组患者(P<0.05);再住院率为13.3%明显低于对照组患者26.7%,差异有统计学意义(P<0.05)。结论在冠心病伴多支血管病变患者的治疗中,PCI能有效降低患者的血清BNP水平,改善患者心功能,进而改善预后。

Objective To explore the clinical effect of percutaneious coronary intervention（PCI）in the cardiac function for coronary heart disease（CHD） patients with multi-vessel disease.Methods Convenient selection through CAG,the clinical data about 60 CHD cases complicated with multi-vessel disease treated from January 2013 to January 2015 in our hospital was retrospectively analyzed.The subjects were divided into observation group and control group according to the possibility of receiving PCI treatment,30 patients in each group.The control group adopted the conventional drug therapy（anti-platelet aggregation,lipid regulation,stabilization of plaque,nitrate,ACEI/ARB,β-receptor blocker）.On the basis,the observation group also adopted PCI on the complete and partial revascularization.The level of BNP,echocardiogram index as well as the re-hospitalization rate caused by repeated myocardial ischemia or heart failure was observed.Results The level of serum BNP（196.17±16.26ng/L）for observation group was significantly lower than that of control group（223.08±15.34）（P〈0.05）;the echocardiogram index for observation group was signficantly better than that of control group（P〈0.05）;the re-hospitalization rate for observation group 13.3% was lower than that of control group 26.7%（P〈0.05）.The difference was statiftically significant.Conclusion For CHD patients complicated with multi-vessel disease,PCI can efficiently reduce the level of serum BNP and improve the patient＇s cardiac function and prognosis.
