预见性护理流程教育对帕金森患者服药依从性的影响

INFLUENCE OF PREDICTIVE NURSINGCARE ON MEDICATION ADHERENCE IN PATIENTS WITH PARKINSON'S DISEASE

目的:探讨预见性护理流程教育对帕金森患者服药依从性及并发症的影响。方法:收集2012年3月至2015年9月陕西省西安医学院第二附属医院收治的帕金森患者1 382例,随机分为观察组和对照组,每组691例。对照组采用常规护理模式,观察组在常规护理基础上,应用建立品管圈、改良护理用具、制定操作指引、组织培训、严密质控等综合措施进行护理干预。分析比较两组患者服药依从性、对疾病的认知程度及"开关现象"、便秘、精神症状等不良反应的发生率。结果:对照组患者服药完全依从率为44.90%,观察组患者为92.04%,两组比较差异有统计学意义(P<0.05);观察组对疾病知识的认知度显著高于对照组(P<0.05);观察组与对照组不良反应总发生率分别为3.76%、20.54%,两组比较,差异有统计学意义(P<0.05)。结论:采用建立品管圈、改良护理用具、制定操作指引、组织培训、严密质控等综合护理措施对提高帕金森患者服药依从性疗效显著,而且明显提高患者对疾病知识的认知度,降低机械通气时间,降低"开关现象"、便秘、精神症状等不良反应的发生率,值得在临床护理工作中推广应用。

Objective To investigate the influence of predictive nursing care on medication adherence in pa-tients with Parkinson＇s disease. Methods ： From March 2012 to September 2015, a total of 1 ,382 patients with Parkinson＇s disease in our hospital were recruited and randomly divided into ana control group with 691 cases in each group. The patients in the control group received routine nursingcare,while those in the observation group received comprehensive nursing measures included establishing quality control circle,improving nursing process, creating operation guidelines, etc . , on the basis of the routine nursing care. Medication adherence, awareness of disease and incidence of adverse reactions be-tween two groups were compared and analyzed. Results： The observation group exhibited a much higher medication adherence compared to the control group （92. 04% vs,44. 90%, P〈0. 05）. Percentage of dis-ease awareness in the observation group was higher than that in the control group （ P〈0.05）.Moreover,the total incidence of adverse reactions in the observation group was lower than that in the control group（3.76% 20. 54% ,P 〈 0. 05）. Conclusion： Comprehensive nursing measures could effectively improvemedication adherence,enhance disease awareness and reduce adverse reactions in patients with Parkinson＇s disease.
