摘要
目的 探讨2型糖尿病肾病早期诊断中检测血清胱抑素C和β2微球蛋白的临床价值。方法 选择2015年1~12月在我院治疗的糖尿病患者142例为研究对象,根据尿白蛋白排泄率分组,A组为单纯糖尿病组,B组为早期肾病组,C组为临床肾病组。另选择健康体检者50例为对照组。比较各组患者血清胱抑素C、β2微球蛋白、BUN、SCr等水平。结果 B组和C组Cys C、β2-MG显著高于对照组和A组(P〈0.01),C组显著高于对照组、A组及B组,C组SCr及BUN显著高于对照组、A组及B组,差异均有统计学意义(P〈0.01)。相关性分析显示,血清Cys C水平与患者的病程、Hb A1C、SBP、β2-MG、SCr、BUN、Malb呈显著正相关的关系(P〈0.01);β2-MG与患者的病程、Hb A1C、SBP、Cys C、SCr、BUN、Malb呈显著正相关的关系(P〈0.01)。结论 2型糖尿病肾病早期血清Cys C及β2-MG就有显著升高,在辅助诊断方面优于BUN及SCr等传统指标。
Objective To discuss the application value of CysC and [3 2-MG in early diagnosis of type 2 diabetic nephropathy. Methods 142 cases patients with type 2 diabetic nephropathy cured in our hospital from Jan to Dec 2015 were selected as the research objects. According to urinary albumin excretion rate, they were divided into simple diabetes group (group A), early nephropathy group (group B), and clinical nephropathy group (group C). 50 cases of healthy volunteer were selected as control group. CysC, β 2-MG, BUN and SCr levels of each group were compared. Results CysC and β2-MG levels of group B and group C were significantly higher than those of group A and control group (P 〈 0.01). CysC and β2-MG levels of group C was significantly higher than those of control group, group A and group B, and SCr and BUN levels of group C was significantly higher than those of control group, group A and group B (P 〈 0.01). Correlation analysis showed that the level of serum CysC was significantly positively correlated with the course of disease, β2-MG, SBP, BUN, SCr, Malb and HbA1C (P 〈 0.01), the level of serum β2-MG was significantly positively correlated with the course of disease, HbAIC, SBP, CysC, SCr, BUN, Malb (P 〈 0.01). Conclusion Serum CysC and β2-MG levels in early type 2 diabetic nephropathy increase are significantly increased and they are better than BUN and SCr and other traditional indicators in the auxiliary diagnosis.
