摘要
目的了解急性冠脉综合征(ACS)患者血浆二十碳五烯酸(EPA)和二十二碳六烯酸(DHA)水平及其与载脂蛋白B100(ApoB100)/载脂蛋白A1(ApoA1)比值关系,为ACS的预防控制提供参考依据。方法于2011年6月—2016年6月整群抽取在黑龙江省牡丹江医学院第二附属医院和牡丹江红旗医院行冠脉造影检查者860人,将其中确诊的610例ACS患者和250名健康者分别作为病例组和对照组进行实验室检测,比较2组人群的血浆EPA、DHA及血清ApoB100、ApoA1和ApoB100/ApoA1比值水平,分析ACS患者血浆EPA、DHA水平及其与ApoB100/ApoA1比值关系。结果病例组患者血浆EPA、DHA和血清ApoA1分别为(3.02±0.25)%、(2.42±0.24)%和(1.21±0.25)g/L,均低于对照组健康者的(3.51±0.32)%、(2.88±0.29)%和(1.42±0.32)g/L,差异均有统计学意义(均P<0.001);病例组患者血清ApoB100和ApoB100/ApoA1比值分别为(1.05±0.23)g/L和(0.87±0.22),均高于对照组健康者的(0.73±0.12)g/L和(0.51±0.16),差异均有统计学意义(均P<0.001);Spearman相关分析结果显示,ACS患者血浆EPA、DHA水平与ApoB100/ApoA1比值均呈负相关(r=-0.762、-0.783,均P<0.05)。
Objective To examine associations of plasma eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) levels with apolipoprotein B100 (ApoB100)/apolipoprotein A1 (ApoA1) ratio among patients with acute coronary syndrome (ACS), and to provide references for ACS prevention and control. Methods A cluster sample of 860 examinees having coronary angiography were selected at The Second Affiliated Hospital of Mudanjiang Medical College and Mudanjiang Hongqi Hospital between June 2011 and June 2016 and 610 of them diagnosed as ACS were assigned into a case group and 250 with negative checkup result into a control group, respectively. Levels of plasma EPA and DHA,serum ApoB100 and ApoA1 ,and the ratio of serum ApoB100/ApoA1 of the participants were determined and compared between the cases and the controls. The relationships between plasma EPA and DHA and serum ApoB100/ ApoA1 ratio were analyzed. Results Significantly lower plasma EPA (3.02 ± 0. 25 % vs. 3.51 ± 0. 32% ), DHA (2.42 ± 0. 24% vs. 2. 88 ± 0. 29% ) and serum ApoA1 ( 1.21 ± 0. 25 g/L vs. 1.42 ± 0. 32 g/L) and higher serum ApoB100 ( 1.05 ± 0. 23 g/L vs. 0. 73 ± 0. 12 g/L) and ApoB 100/ApoA1 ratio ( 0. 87 ± 0. 22 vs. 0. 51 ± 0. 160. 51 ± 0. 16 ) were observed in the cases than in the controls ( all P 〈 0. 001 ). Spearman correlation analyses revealed significantly reverse correlations between ApoB100/ApoA1 ratio and plasma EPA ( r = - 0. 762 ) and DHA ( r = - 0. 783 ) among the ACS cases (both P 〈 0. 001 ). Conclusion Plasma EPA and DHA and serum ApoB100/ApoA1 ratio may be indicators for the diagnosis and prognosis of patients with acute coronary syndrome.
