2013—2015年丹东市中医院门、急诊抗菌药物应用分析

Analysis on Application of Antibioticsin Outpatient and Emergency Department of Dandong Traditional Chinese Medicine Hospital During 2013-2015

目的:了解丹东市中医院(以下简称"我院")门、急诊抗菌药物使用情况,为加强抗菌药物科学管理及合理应用提供参考。方法:调取2013—2015年我院门、急诊处方402 314张,以《处方管理办法》《抗菌药物临床应用指导原则》等为依据,对抗菌药物使用率、品种选择、联合用药情况、用药频度等进行统计分析,对存在的问题提出改进建议。结果:402 314张处方中,抗菌药物处方56 324张,抗菌药物使用率为14.00%,其中儿科抗菌药物使用率较高(25.18%,14 181张处方);抗菌药物多用于呼吸系统疾病,其中肺炎居首位,多为单一用药;使用的抗菌药物以头孢菌素类为主,共7个品种、42 898例次,用药频度最高的为头孢哌酮舒巴坦钠;抗菌药物给药途径以静脉给药为主;不合理用药主要为用药指征不明确。结论:我院门、急诊抗菌药物的使用基本合理,仍存在一定问题,应加强对抗菌药物临床应用知识的培训和监管。

OBJECTIVE： To investigate the application status of antibioticsin outpatient and emergency department of Dandong Traditional Chinese Medicine Hospital（ hereinafter referred to as＂our hospital＂）,so as to provide reference for the scientific management and rational application of antibiotics. METHODS： 402 314 prescriptions of outpatient and emergency department of our hospital during 2013-2015 were extracted. According to thebasis of ＂Prescription Management Method＂and＂Guiding Principle of Clinical Application of Antibiotics＂,statistical analysis was conducted on the application rate,categories selection,drug combination and defined daily dose system（ DDDs） of antibiotics,some suggestions were put forward to improve the existing problems. RESULTS： Of the 402 314 prescriptions,56 324 cases were antibiotics prescriptions,the application rate was 14. 00%; among which the application rate of antibiotics in pediatrics took the lead（ 25. 18%,14 181prescriptions）. Antibiotics often used in respiratory system disease,mainly for pneumonia assingle drug. Cephalosporins were used most,which had 7 varieties,including 42 898 times; DDDs of cefoperazone-sulbactam sodium took the lead; intravenous administration was the main route of administration; and irrational prescription was mainly in no indication. CONCLUSIONS： The application of antibioticsino utpatient and emergency department of our hospital is basically rational,yet there are still some irrational phenomenon,it is necessary to strengthen the supervision and train of clinical application of knowledge of antibiotics.
