HPV分型及梅毒与尖锐湿疣复发的相关性研究

目的:研究HPV分型及梅毒与尖锐湿疣(CA)复发的相关性研究,旨在为临床上降低尖锐湿疣的复发率提供数据资料。方法:选取2010年1月至2015年6月于我院皮肤性病科就诊并首次行HPC分型检测的尖锐湿疣患者110例,分析HPV分型与尖锐湿疣的复发以及HPV分型及梅毒与尖锐湿疣复发的相关性。结果:单纯感染HPV6/11亚型的患者最多,有84例,占比76.36%;感染HPV16/18亚型有17例,占比15.45%;多种混合感染有9例,占比8.18%。单纯感染HPV6/11的84例患者中有40例患者出现复发,占比47.62%;而HPV16/18型感染的患者中17例患者有3例出现复发,占比17.65%;多重混合感染中有4例患者出现复发,占比44.44%,三组患者复发构成比有显著差异(P<0.05)。在HPV6/11型患者中,并发梅毒感染的患者复发率占比与无梅毒感染的尖锐湿疣患者复发率有显著差异(P<0.01);HPV16/18型患者中,并发梅毒感染的患者复发率与无梅毒感染的患者复发率没有显著差异(P>0.05);在多重混合感染的患者中,并发梅毒感染的患者复发率与无梅毒感染患者的复发率有显著差异(P<0.05)。结论:低危型的HPV6/11亚型感染是尖锐湿疣患者中最常见的感染,而且也更易导致患者尖锐湿疣复发;合并梅毒感染的尖锐湿疣患者也更易于导致尖锐湿疣复发,值得在临床上加以重视。

Objectives： To investigate HPV typing and the correlation of syphilis and condyloma acuminatum（ CA） relapse,in order to reduce the recurrence of CA in clinical. Methods： 110 CA patients in our hospital from January 2010 to June 2015 for HPV genotyping for the first time were selected. The correlation of HPV genotyping and the recurrence of CA as well as the correlation of HPV typing and syphilis and CA recurrence was studied.Results： 84 patients were infected by HPV6 / 11 subtypes,accounting for 76. 36%,with 40 cases of recurrence（ 47. 62%）; 17 patients were infected by HPV16 / 18 subtypes,accounting for 15. 45%,with 3 cases of recurrence（ 17. 65%）; 9 patients were infected by a variety of subtypes,accounting for 8. 18%,with 4 cases of recurrence（ 44. 44%）. There was significant difference in the recurrence rate among the three groups（ P〈0. 05）. In HPV6 / 11- type infected patients,the recurrence of patients with syphilis had significant difference from the recurrence of patients without syphilis infection（ P〈0. 01）,while such difference in HPV16 / 18- type infected patients was not significant（ P〉0. 05）,but significant in patients with multiple infection（ P〈0. 05）. Conclusion：The low- risk type of HPV6 / 11 CA subtype infection is the most common infection in CA patients,which is also more likely to lead to relapse of CA. CA patients with syphilis are also easier suffering from recurrence of CA,which deserves to be valued in clinical.
